BPC-157 vs TB-500
The short answer
BPC-157 and TB-500 are two different unapproved peptides sold for injury healing, and neither has been shown to heal injuries in people12. The five small published studies of BPC-157 that FDA found gave it to fewer than 100 people, for 2 weeks or less, while FDA found no study that gave TB-500 to people, and neither did our PubMed search123. In a 2026 rat study of Achilles tendon repair that tested both, the peptide its authors called synthetic thymosin beta-4 (TB-500) alone made tendons stronger, BPC-157’s gains were not statistically significant, and combining them added no benefit4.
At a glance
| Compared | BPC-157 | TB-500 |
|---|---|---|
| Evidence tier | B At least one controlled human trial | D Animal studies only |
| Does it work? | For tendon and joint healing: UntestedFDA found no controlled human trial of it for tendons or joints; those claims rest on animal studies15. The only controlled trial in patients that FDA found, for a bowel disease, showed no clear benefit1. | For tendon, ligament, muscle and bone healing: UntestedNeither FDA’s review nor our PubMed search found a study in people23. Healing claims rest on a few animal and cell studies with mixed results; the tendon study called its peptide synthetic thymosin beta-4246. |

| What it is | A 15-amino-acid peptide described as a fragment of a protein from human gastric juice112. | A 7-amino-acid fragment (positions 17 to 23) of thymosin beta-4, a 43-amino-acid protein; FDA says the two are not the same substance2. |
| How it might work | Unknown; animal and cell studies suggest new blood-vessel growth, nitric oxide production and dampened inflammation113. | Unknown; in a cell test, TB-500 itself did not speed the closing of scratch wounds, while one of its breakdown products had a small effect26. |
| Studied in people for | Ulcerative colitis (as an enema), knee pain, bladder pain and IV safety, in the published studies FDA found, which gave it to fewer than 100 people in all; an 8-week study of gummies has posted no results, and a trial after rotator cuff surgery is not due to start until January 202711415161718. | Nothing: FDA found no clinical trial, case report or other human data for TB-500 by any route2. |
| Best human evidence | One randomized trial, in 53 people with ulcerative colitis, which found no statistically significant benefit1. | None; trials of full-length thymosin beta-4, such as a 700-person eye-drop study, tested a different molecule219. |
| Biggest documented risk | Unknown human safety; in 28-day animal studies it changed blood clotting and raised liver-related blood levels120. | FDA found no human or animal safety data, including no cancer studies; a possible cancer signal dominated discussion at FDA’s 2026 panel, according to the FDA Law Blog221. |
| Lab tests of gray-market vials | 92 of Finnrick’s latest 200 tests failed (27 May to 25 September 2026), and a bodega vial tested for WIRED held no BPC-1572223. | 113 of Finnrick’s latest 200 tests failed (29 January to 25 September 2026), and a 2023 lab study found contents did not always match product descriptions2425. |
Is there evidence for taking them together?
FDA’s reviews found no human study of BPC-157 and TB-500 together12. In a 2026 study of rats with repaired Achilles tendons (8 per group), TB-500 alone gave stronger tendons, BPC-157’s gains were not statistically significant, and combining the two added no benefit over either alone4. The authors called the study exploratory and described their TB-500 as ‘synthetic thymosin beta-4’, so it may have been the full protein rather than the fragment, and earlier rat work had reported faster Achilles healing with BPC-157 alone426.
The nearest human data come from a 2021 chart review at a Florida clinic: 3 of 4 knee-pain patients given BPC-157 plus ‘TB4’ said their pain improved, but there was no control group14. The paper and FDA’s BPC-157 review both name the second peptide as thymosin beta-4, not TB-500114.
FDA has also described a report from a 40-year-old woman who injected a BPC-157 and TB-500 ‘research purposes only’ product: her skin and gums darkened, the reaction returned when she used it again, and FDA could not tell which peptide was responsible1.
What is the ‘Wolverine stack’?
‘Wolverine stack’ is a popular name for BPC-157 paired with TB-500, and at least one seller offers both in a single vial holding 10 mg of each2728. Supporters described the pairing to FDA’s advisory panel in July 202621.
Other blends add more peptides: In December 2025 Health Canada reported seizing products sold as ‘Glow’ (BPC-157, TB-500 and GHK-Cu) and ‘Klow’ (the same plus KPV)29. FDA also found nasal sprays that combine BPC-157 with a thymosin beta-4 fragment1.
Interest is growing: ‘wolverine stack’ was among the rising US Google searches related to BPC-157 in the year to 29 September 2026, and a health-records analysis reported by Reuters found new BPC-157 users up 33-fold from 2020 to 2026, in a preprint that has not been peer reviewed2830.
Questions people ask
What are the differences between TB-500 and BPC-157?
They are different molecules: BPC-157 is a 15-amino-acid peptide described as a fragment of a gastric-juice protein, while TB-500 is a 7-amino-acid fragment of thymosin beta-412. BPC-157 has five small human studies, and FDA found no human data at all for TB-50012. Neither is an approved drug, and both are banned in sport129.
Should I take TB 500 with BPC-157?
FDA’s reviews found no human study of the pair, and in a 2026 rat tendon study adding BPC-157 to TB-500 gave no extra benefit24. FDA has a report of skin and gum darkening in a woman who injected a combined product, and it could not tell which peptide caused it1. Neither peptide is an approved drug, and US pharmacies may not compound either1278.
Which is better for tendon injuries, BPC-157 or TB-500?
FDA found no controlled human trial of either for a tendon injury, and a BPC-157 trial after rotator cuff surgery is not due to start until January 20271218. In rats, earlier work reported faster Achilles healing with BPC-157, but a 2026 rat study found that only TB-500, which its authors called synthetic thymosin beta-4, significantly improved tendon strength426.
Has TB-500 been tested in people?
No: FDA’s 2026 review found no clinical trial, case report or other human data for TB-500 by any route, and a PubMed search on 8 October 2026 found no study either23. Trials of full-length thymosin beta-4, a different molecule, include a 700-person eye-drop study that, its maker said, did not meet its main goals; results posted on 28 August 2026 show similar average changes on the drops and on placebo1931.
