Survodutide dosage: what studies actually used
The short answer
No dose of survodutide is approved, because no regulator has approved it as of 7 October 202612. In the phase 3 SYNCHRONIZE trials, the dose was raised step by step to a target of 3.6 mg or 6.0 mg once a week, injected under the skin, over 76 weeks34. This page reports trial doses and is not dosing advice. Vials sold online are another matter: of the 27 survodutide tests Finnrick listed from April 2025 to April 2026, 13 failed and 1 was incomplete5.

Studies, animals and users: kept apart
Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.
In human studies
No dose is approved. In the Phase 3 SYNCHRONIZE-1 and SYNCHRONIZE-2 trials, the dose was raised gradually to a target of 3.6 mg or 6.0 mg once weekly, injected under the skin, for 76 weeks in total34. SYNCHRONIZE-MASLD used 6.0 mg weekly for 48 weeks10. The Phase 2 obesity trial tested 0.6, 2.4, 3.6 and 4.8 mg weekly over 46 weeks8, and the Phase 2 MASH trial tested 2.4, 4.8 and 6.0 mg weekly over 48 weeks9. A 16-week Phase 2 trial in type 2 diabetes tested doses up to 2.7 mg weekly or 1.8 mg twice weekly7.
In animal studies
Mice: diet-induced obese male mice received daily injections under the skin of 3, 10, 20 or 30 nmol/kg for 30 days, compared with semaglutide at 20 or 100 nmol/kg12. These are research doses in animals, not human doses.

Why the dose goes up slowly
In SYNCHRONIZE-1 and SYNCHRONIZE-MASLD, digestive side effects were most common while the dose was going up610. In SYNCHRONIZE-1, they led 17.8% of people on 3.6 mg and 20.2% on 6.0 mg to stop, versus 2.9% on placebo6. In SYNCHRONIZE-2, 18% on survodutide stopped because of them, versus 1.2% on placebo1. Boehringer Ingelheim says these stops usually came during the step-up phase, as the trial protocol allowed limited flexibility to manage side effects, and that current and upcoming trials allow more flexible step-ups1.
Earlier trials used different paces8911. In phase 1, weekly doses were raised over 6 weeks in one part and over 16 weeks in another11. The phase 2 obesity trial used 20 weeks of step-up before 26 weeks of maintenance, and the phase 2 MASH trial a fast 24-week step-up before 24 weeks of maintenance89.
Why each phase 3 trial reports two results
Phase 3 results come in two versions36. The treatment-regimen analysis includes the effects of stopping early, using obesity drugs the protocol did not allow, and a longer step-up; the efficacy analysis models what happens if everyone follows the protocol36. In SYNCHRONIZE-1, weight fell 12.2% and 13.0% by the first and 15.3% and 16.6% by the second, versus 5.4% and 3.2% on placebo36.
The placebo group lost more weight than planned for, 5.4% versus the 2% to 3% assumed, and more than 16% of it reported using a prohibited GLP-1-based medicine6. Boehringer Ingelheim’s headline of up to 13.1% weight loss in SYNCHRONIZE-2, versus 3.1% on placebo, uses the efficacy analysis1.
Assigned dose versus dose reached
The phase 2 obesity trial grouped people by the dose assigned at the start, and by that measure weight fell by up to 14.9% on 4.8 mg8. Only 233 of 386 people (60.4%) completed the 46 weeks of treatment8. Zealand Pharma, which licensed survodutide to Boehringer Ingelheim, shows an analysis by the dose people reached at the end of treatment: up to 18.7% on 4.8 mg, in a group of 54 people, while 77 had been assigned to that dose813.
What the higher dose adds, and what it costs
In SYNCHRONIZE-1, weight fell 13.0% on 6.0 mg and 12.2% on 3.6 mg, while digestive side effects affected 89.7% and 80.9%3. In SYNCHRONIZE-2, in people with type 2 diabetes, weight fell 9.8% and 8.2%, while digestive side effects affected 77.7% and 72.8%4.
In the phase 2 MASH trial, the middle dose did best: MASH improved in 62% on 4.8 mg, versus 47% on 2.4 mg and 43% on 6.0 mg, and a curved (quadratic) dose-response fitted best9.
What about a missed dose?
Survodutide has no label, so there is no official rule for a missed dose1. Boehringer Ingelheim says its efficacy and safety have not been established1.
Why gray-market vials make doses unreliable
FDA says companies have illegally sold unapproved GLP-1 drugs, including survodutide, falsely labeled “for research purposes”, directly to consumers16. As of 7 October 2026, FDA’s warning-letter database listed two letters naming survodutide, dated 10 December 2024 and 24 August 2026171819.
Finnrick, an independent testing service, listed 27 survodutide tests dated 25 April 2025 to 23 April 2026: 13 passed, 13 failed and 1 was incomplete, as displayed on 7 October 20265. Its pass mark allows the measured dose to differ from the label by up to 35%5.
Questions people ask
How much survodutide should I take?
No dose is approved, because survodutide is not approved anywhere as of 7 October 202612. Phase 3 trials raised the dose step by step to 3.6 mg or 6.0 mg once a week34. Ask your prescriber about approved options.
What is the highest dose of survodutide tested?
6.0 mg once a week is the highest dose in the phase 2 and phase 3 trials on this page, used in SYNCHRONIZE-1, SYNCHRONIZE-2, SYNCHRONIZE-MASLD and the phase 2 MASH trial34910. The phase 1 trial’s published summary does not give its dose levels11.
Is survodutide 6 mg better than 3.6 mg?
In both phase 3 trials, weight fell more on 6.0 mg than on 3.6 mg at 76 weeks: 13.0% versus 12.2% in SYNCHRONIZE-1, against 5.4% on placebo, and 9.8% versus 8.2% in SYNCHRONIZE-2, against 3.9%34. Digestive side effects were also more common on 6.0 mg in both trials34.
How long did the survodutide step-up take in trials?
It varied: 20 weeks in the phase 2 obesity trial, and 24 weeks in both the phase 2 MASH trial and the phase 3 SYNCHRONIZE-MASLD trial, which raised the dose to 6.0 mg8910. The MASH trial’s authors called its 24-week step-up rapid, and Boehringer Ingelheim says current and upcoming trials allow more flexible step-ups19.
