Oxytocin dosage: what studies actually used
The short answer
Oxytocin is a hormone, and in the US approved oxytocin is sold only as injections, such as Pitocin, which is used in hospitals to start or strengthen labor and to control bleeding after birth34. To start or strengthen labor, the label says it should be given only as a drip into a vein, with medical supervision in a hospital4. Nasal-spray research has mostly used single doses of 18 to 40 IU (international units), and a 24-week trial that targeted 48 IU a day in children with autism found no benefit over placebo56. No nasal dose is approved in the US, and sprays and vials sold online are not FDA-approved products37. This page reports those doses and is not dosing advice.

Every dose studied in people
| Study | Who | People | Dose and route | What happened |
|---|---|---|---|---|
| CONDISOX (Boie 2021), double-blind randomized trial in 10 hospitals | Women whose induced labor was being stimulated with an oxytocin drip | 1,200 | The drip was either continued or stopped once active labor began; the abstract gives no rates Drip into a vein, in hospital · During labor |
Cesarean rates were 16.6% when the drip was stopped and 14.2% when it was continued, a difference that was not statistically significant (relative risk 1.17, 95% CI 0.90 to 1.53)8. Stopping it cut uterine hyperstimulation, meaning overly strong or frequent contractions, from 12.9% to 3.7%, but labor lasted longer (median 282 versus 201 minutes)8. |
| Adnan 2018, double-blind randomized trial | Women aged 18 or older at term with one baby, aiming for a vaginal delivery | 1,075 | 10 IU once at delivery, into a vein or into a muscle, with a placebo injection by the other route Injection into a vein or a muscle, in hospital · Single dose |
Of 1,075 women randomized, 1,035 were analyzed9. Bleeding of 500 mL or more was not significantly less common with the vein route (18.8% versus 23.2%), but severe bleeding of 1,000 mL or more was (4.6% versus 8.1%), and side effects were similar (4.1% versus 5.2%)9. |
| Nordström 1997, double-blind placebo-controlled randomized trial | Women having a vaginal delivery of one baby, head first | 1,000 | 10 IU once, between the birth and the delivery of the placenta, versus saline Injection into a vein, in hospital · Single dose |
Mean total blood loss was 407 mL with oxytocin versus 527 mL with saline, and extra treatment with another contraction drug was needed in 7.8% versus 13.8%10. |
| SOARS-B (Sikich 2021), randomized placebo-controlled Phase 2 trial | Children and teens aged 3 to 17 with autism | 290 | Target total of 48 IU a day Nasal spray · 24 weeks |
Social withdrawal scores improved by 3.7 points with oxytocin and 3.5 with placebo, with no significant difference (P = 0.61), and adverse events were similar in incidence and severity6. |
| Yamasue 2020, randomized placebo-controlled double-blind trial in Japan | Adults aged 18 to 48 with autism | 106 | 48 IU a day Nasal spray · 6 weeks |
The main social-reciprocity score improved on both oxytocin and placebo, with no difference between them (P = .69), although repetitive behavior improved more with oxytocin11. Adverse events were no more common than on placebo, and one participant had temporary breast enlargement (gynecomastia)11. |
| Guastella 2023, randomized placebo-controlled double-blind trial | Children aged 3 to 12 with autism | 87 | 16 IU morning and night (32 IU a day), after a 3-week placebo lead-in Nasal spray · 12 weeks |
There was no overall effect on caregiver-rated social responsiveness (p = 0.686), with some sign of benefit only in children aged 3 to 51. It was well tolerated, and more side effects were reported on placebo1. |
| Daniels 2023, randomized placebo-controlled double-blind trial | Children aged 8 to 12 with autism, 61 boys and 16 girls | 77 | 12 IU twice a day Nasal spray · 4 weeks |
Social responsiveness improved in both groups, and the improvement was not specific to oxytocin2. |
| Plessow 2024, randomized placebo-controlled double-blind trial | Adults with obesity | 61 | 24 IU four times a day Nasal spray · 8 weeks |
Weight did not change differently from placebo (0.20 versus 0.26 kg, P=0.934), and neither did body fat or resting energy use, although intake at one test meal fell by 152.0 kcal compared with placebo12. There were no serious adverse events12. |
| Behnia 2014, study of couples in a natural setting | Healthy heterosexual couples (29 couples) | 58 | One 24 IU dose Nasal spray · Single dose |
Sexual drive, arousal, erection and lubrication did not change, while orgasm intensity and contentment after intercourse rose, with small to moderate effect sizes13. |
| Heinrichs 2003, placebo-controlled double-blind study | Healthy men | 37 | One 24 IU dose, 50 minutes before a stress test Nasal spray · Single dose |
Oxytocin eased anxiety during a lab stress test, and oxytocin plus support from a best friend gave the lowest levels of the stress hormone cortisol14. |
| Muin 2015, randomized placebo-controlled double-blind crossover trial | Premenopausal and postmenopausal women with sexual dysfunction | 30 | 32 IU on demand, within 50 minutes before intercourse Nasal spray · 8 weeks on each treatment, with a 2-week break between |
Sexual function scores rose 26% on oxytocin and 31% on placebo, with no significant treatment effect15. |
| Guastella 2009, randomized placebo-controlled double-blind trial | People with social anxiety disorder having exposure therapy | 25 | 24 IU with exposure-therapy sessions Nasal spray · Not stated in the abstract |
Self-ratings of appearance and speech performance improved, but overall symptom relief was similar with oxytocin and placebo16. |
| Ellenbogen 2024, pilot randomized placebo-controlled double-blind trial | Volunteers with major depressive disorder having interpersonal therapy | 23 | 24 IU before each of 16 therapy sessions Nasal spray · 16 therapy sessions, then 6 months of follow-up |
Depressive symptoms fell faster with oxytocin than with placebo (p < 0.05), with medium-to-large effects after treatment and at follow-up, but the authors called it a pilot to be repeated in larger samples17. |
| Striepens 2013, placebo-controlled study of blood and spinal fluid levels | People given oxytocin (11) or placebo (4) | 15 | One 24 IU dose Nasal spray · Single dose |
Levels rose in the blood, peaking at 15 minutes, and in the fluid around the brain and spinal cord, where they took up to 75 minutes to rise significantly, with no correlation between the two18. |
Studies, animals and users: kept apart
Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.
In human studies
Pitocin label: to start or strengthen labor, only as a drip into a vein, in hospital and under medical supervision, started very low and adjusted to how the uterus responds; after delivery, to control bleeding, as a drip into a vein or 10 units into a muscle after the placenta is delivered4. Nasal research has mostly used single doses of 18 to 40 IU, with up to 182 doses in some studies5. Longer trials used 32 IU before intercourse for 8 weeks15, a target of 48 IU a day for 24 weeks in children with autism6, and 24 IU four times a day for 8 weeks in adults with obesity12.
In animal studies
Female prairie voles: mini-pumps released 1 to 100 ng of oxytocin per hour into the brain’s ventricles or under the skin during 6 hours with a male; 10 and 100 ng per hour into the brain produced a partner preference, while delivery under the skin did not20.
What users report Anecdotal · not validated · not a recommendation
A dosing-protocol website (updated 27 September 2026) lists 100 to 500 µg a day, injected under the skin or sprayed into the nose19.
Half-life and how it is taken
- Half-life
- about 1 to 6 minutes4Plasma half-life of injected oxytocin, per the Pitocin label; shorter in late pregnancy and during breastfeeding
- Routes
- Intravenous drip or injection: the approved hospital route for labor and after delivery; large human trials
- Intramuscular injection: approved after delivery of the placenta; large human trials
- Nasal spray: many controlled studies, mostly of single doses, including one of 24 weeks; Syntocinon nasal solution is discontinued in the US, and telehealth companies sell compounded sprays
- Lozenge (troche): sold by a telehealth company; no controlled trial found
- Injection under the skin: listed by a dosing website for ‘research’ vials; no human trial of this route found
How Pitocin is used in hospitals
Pitocin’s label covers starting labor when there is a medical reason, strengthening labor, add-on treatment of incomplete or inevitable abortion, and producing contractions and controlling bleeding after birth4. It is not indicated for elective induction, meaning starting labor in someone with no medical reason for it4.
The label says the dose is set by how the uterus responds, so it must be individualized and started at a very low level4. Everyone receiving it by vein must be under continuous observation by trained staff, with a physician qualified to manage complications immediately available4. Its drip rates are written for hospital teams, who adjust them to each patient’s contractions while monitoring mother and baby4.
The label warns that overstimulating the uterus can harm both mother and baby, and that because oxytocin makes the body hold on to water, water intoxication with seizures can occur, especially when it is given continuously by drip4. For bleeding after birth, a 2019 Cochrane review found that preventive oxytocin may reduce blood loss of 500 mL or more compared with no drug or placebo (risk ratio 0.51; 6 studies, 4,162 women), and it called for more high-quality trials of the best dose and route21.
Nasal-spray doses in research
Nasal studies state their doses in IU5. A 2011 review of 38 randomized trials, with 1,529 participants, found side effects no different from placebo in short-term use in controlled research settings5.
The longer trials in the table mostly missed their main goals, in autism, obesity and sexual problems6111215. How much of a nasal dose reaches the brain is also unclear: a 2016 review noted that very little of the large amounts sprayed into the nose appears to reach the fluid around the brain, and called for proper dose-response studies22.
Sprays and vials sold online
No oxytocin nasal spray is approved and on the market in the US: Syntocinon nasal solution, the one nasal product in FDA’s drug records, is listed as discontinued3. Telehealth companies sell nasal sprays and lozenges made by compounding pharmacies, and FDA does not approve compounded drugs or check their safety, effectiveness or quality before they are sold72324.
‘Research’ vials of oxytocin powder are sold by weight, in mg, and labeled not for human use, while the studies above state their doses in IU2526. The testing service Finnrick’s public summary, generated on 25 September 2026, listed no tests of oxytocin, so what these products contain is unverified27.
Overuse can be dangerous: a 1985 case report described a nursing mother who used an oxytocin nasal spray excessively while receiving a large amount of fluid by vein and developed severe water intoxication with seizures, and the authors judged the spray the likely cause28.
Questions people ask
How much oxytocin should I take?
No dose is established for taking it on your own: approved US oxytocin, such as Pitocin, is an injection given in hospitals by clinicians, for labor and after birth34. Nasal studies mostly used single doses of 18 to 40 IU, and the longer trials mostly missed their main goals5612. Any decision about using it belongs with a licensed prescriber.
What is the dose of oxytocin nasal spray?
No nasal dose is approved in the US, and the one nasal product in FDA’s records is discontinued3. Research sprays mostly gave single doses of 18 to 40 IU, while longer trials gave 24 IU four times a day for 8 weeks in adults with obesity and targeted 48 IU a day for 24 weeks in children with autism, without benefit over placebo5612.
What dose of Pitocin is used during labor?
To start or strengthen labor, the label says Pitocin should be given only as a drip into a vein, with adequate medical supervision in a hospital4. The dose is set by how the uterus responds, individualized and started at a very low level, and trained staff watch the patient continuously4.
Does a higher dose of oxytocin work better?
There is no good evidence that it does: a review of 9 schizophrenia trials found no consistent benefit, and a hint of benefit at doses above 40 to 80 IU disappeared when one outlier study was left out29. Reviewers have called for proper dose-response studies22.
Can you take too much oxytocin?
Yes. The Pitocin label warns that overstimulating the uterus can harm mother and baby, and that water intoxication with seizures can occur4. A 1985 case report described a nursing mother who overused a nasal spray and developed severe water intoxication with seizures28.
