Sermorelin vs Tesamorelin

The short answer

Both copy GHRH, the brain hormone that tells the pituitary to release growth hormone, but tesamorelin has much stronger evidence, for one use: two placebo-controlled trials in 816 adults with HIV found it cut deep belly fat, and FDA approved it for that alone3456. Sermorelin’s trials are older and smaller, mostly in children who make too little growth hormone, where growth hormone itself did as well or better, and its approval was withdrawn in 2009 after its maker stopped selling it7891011. We found no trial that compared the two directly12. Each has documented downsides: tesamorelin pushed IGF-1 more than 2 standard deviations above the normal average in 47% of trial patients, and sermorelin triggered antibodies in 39 of 40 children in one trial310.

Does it work? For anti-aging: Mixed resultsIn a 6-month trial against placebo in 89 older adults, some thinking tests improved12. The team’s other reports found less body fat but no better sleep, and said routine use was not yet supported1314.For reducing deep belly fat: Mixed resultsIn two large HIV trials, deep belly fat fell about 15% more than on placebo, then returned after stopping4615. Outside HIV, a 12-month trial lowered it, in people with obesity and low growth hormone output16.

What it isA lab-made copy of the first 29 of GHRH’s 44 amino acids, capped with an amide group2930.A lab-made copy of all 44 amino acids of GHRH, with a short fatty chain added to the first that makes it more stable33132.
Tested in people forGrowth in children who make too little growth hormone, a diagnostic test of the pituitary, and aging in healthy older adults81217.Deep belly fat and liver fat in adults with HIV, plus smaller trials in type 2 diabetes, healthy men and older adults with or without mild memory problems631333435.
Largest trialAn open study (no comparison group) of 110 children who make too little growth hormone: average growth rose from 4.1 to 8.0 cm a year at 6 months in the 86 analyzed8.A placebo-controlled trial of 412 adults with HIV and excess belly fat, lasting 26 weeks: besides cutting deep belly fat, it lowered triglycerides, a blood fat, by 50 mg/dL vs a 9 mg/dL rise on placebo6.
Effect on IGF-1In 10 older men, 14 days of twice-daily injections raised IGF-1, a growth factor, to young-adult levels at the higher of two doses, but 6 weeks of nightly injections did not raise it in 11 older men in another study3637.Rose 81% in the first large trial; after 26 weeks, 47% of patients had IGF-1 more than 2 standard deviations above the normal average36.
Biggest documented riskFlushing and injection-site pain were the most common side effects in children; FDA’s database holds unverified reports coded as anaphylaxis, a severe allergic reaction3839.High IGF-1, which European reviewers called a major safety concern because it may raise cancer risk; HbA1c, a long-term blood sugar measure, reached the diabetes range in 5% vs 1% on placebo; joint pain and limb swelling340.
Antibodies39 of 40 children in one trial made antibodies; they had almost disappeared 9 months after stopping and did not track with growth10.In trials, antibodies to it were found in 50% of patients treated for 26 weeks and 47% of those treated for 52 weeks; about 60% of those with antibodies also reacted with the body’s own GHRH3.
Product quality problemsFDA lists 92 recalled sermorelin products from 2012 to 2025, 81 for sterility problems; 27 of 48 completed tests by Finnrick, an independent testing service, failed (20 April 2025 to 14 September 2026)4142.83 of 191 completed tests among Finnrick’s latest 200 failed (5 May to 24 September 2026)43.

Why is one approved and the other not?

Sermorelin came first: FDA approved it as Geref in 1990 for a pituitary test and in 1997 to treat children with growth failure from growth hormone deficiency717. Its maker stopped selling both products in 2008, FDA withdrew the approvals effective 18 June 2009, and in 2013 FDA found that the withdrawal was not for reasons of safety or effectiveness711.

Tesamorelin was approved as Egrifta on 10 November 2010, after two Phase 3 trials with 412 and 404 adults with HIV, for a single use: reducing excess belly fat in adults with HIV and lipodystrophy, an abnormal change in where the body stores fat345618. Its labels say it is not for weight loss and that its long-term heart safety is not established318. In Europe, Ferrer withdrew its application in 2012, when regulators’ provisional view was that the fat loss had not been shown to bring real health benefits and that raised IGF-1 was ‘a major safety concern’40.

What happened when each was tested in older adults?

Both were tested in older adults at the University of Washington, and the two trial reports share authors123344. The sermorelin trial gave 89 healthy adults, average age 68, daily GHRH or placebo for 6 months; the paper does not name the drug, but the team’s registry record and a 2001 report point to sermorelin121444. The GHRH group scored better on 4 thinking tests, including a performance IQ score (each p<0.01), and on a dual-task test (p<0.04)12.

The tesamorelin trial gave 152 adults aged 55 to 87, 66 of them with mild cognitive impairment (memory and thinking problems short of dementia), daily tesamorelin or placebo for 20 weeks33. Thinking scores improved modestly (P=.03), most clearly in executive function, the mental skills used to plan and switch between tasks33. IGF-1 rose 117% but stayed within the normal range, and percent body fat fell 7.4%, although fat loss was not what the trial set out to test33. Mild side effects were reported by 68% on tesamorelin vs 36% on placebo33. A 2025 open-label trial in 73 people with HIV found no clear thinking benefit over usual care; its authors note it lacked statistical power and a placebo group45.

The tesamorelin authors called for longer trials, and in 2003 the sermorelin investigators wrote that the results did not yet support routine use of GHRH in normal aging1333.

Questions people ask

Is tesamorelin better than sermorelin?

Tesamorelin has much stronger evidence, but only for one use: it cut deep belly fat in large placebo-controlled trials in adults with HIV, while sermorelin’s largest trial was an open study of 110 children815. We found no trial that compared them directly12. Tesamorelin’s risks are also better documented: after 26 weeks, 47% of trial patients had IGF-1 more than 2 standard deviations above the normal average3.

Is sermorelin the same as tesamorelin?

No: both are lab-made versions of GHRH, but sermorelin is its first 29 amino acids, while tesamorelin keeps all 44 and adds a short fatty chain329. They were tested in different people: sermorelin mainly in children who make too little growth hormone, tesamorelin mainly in adults with HIV and excess belly fat815.

Which is better for belly fat, sermorelin or tesamorelin?

Only tesamorelin has been shown to cut deep belly fat in large trials, and only in adults with HIV; the fat returned after people stopped4. For sermorelin, the closest data come from older adults: early results from a 6-month trial showed percent body fat falling about 5% in men and in women not taking estrogen, while a 6-week study found no change in fat on body scans1437. Tesamorelin’s labels say it is not for weight loss318.

Can you take sermorelin and tesamorelin together?

No published or registered study has tested them together12. Both act on the same target, the pituitary’s GHRH receptor1846. Of the two, only tesamorelin is FDA-approved, and only to reduce excess belly fat in adults with HIV and lipodystrophy3518.
