Orforglipron vs semaglutide

The short answer

ClinicalTrials.gov lists no trial that compares the two drugs as weight-loss treatments; the only head-to-head trial, ACHIEVE-3, was in adults with type 2 diabetes12. In it, daily orforglipron lowered blood sugar and weight more than 7 and 14 mg semaglutide tablets, the Rybelsus doses, but caused more digestive side effects, and Lilly, which makes orforglipron, funded it245. In separate placebo-controlled trials, the top Foundayo dose cut weight by 11.2% over 72 weeks and the 25 mg Wegovy pill by 13.6% over 64 weeks, results that cannot be compared directly678. The clearest difference is how they are taken: Foundayo with or without food, the Wegovy pill on an empty stomach with up to 4 ounces of water and nothing else for 30 minutes79.

What it isA small lab-made molecule, not a peptide, that switches on the GLP-1 receptor; about 77% of a 0.8 mg dose reaches the bloodstream725.A modified version of the human gut hormone GLP-1, a peptide with 94% the same sequence; about 1% to 2% of a tablet dose reaches the bloodstream, against 89% of an injection9.
Pill brands and approved uses in the USFoundayo, approved on 1 April 2026 to reduce excess weight and keep it off in adults with obesity, or overweight with at least one weight-related condition713. As of 6 October 2026 it was not approved in the US for type 2 diabetes714; the UK authorized it for both uses on 10 August 202626.Wegovy tablets, approved on 22 December 2025 to reduce excess weight and to lower the risk of heart attack, stroke and cardiovascular death in adults with heart disease and obesity or overweight915. Rybelsus and Ozempic tablets are approved for type 2 diabetes5.
How the pill is taken (label)Once a day, with or without food; swallowed whole7.Once a day in the morning on an empty stomach, with up to 4 ounces of water; then no food, drink or other pills for at least 30 minutes9.
Dose steps on the label0.8 mg a day to start, then 2.5 mg and 5.5 mg, each after at least 30 days; it may go up to 9, 14.5 and at most 17.2 mg7.1.5 mg a day for 30 days, then 4 mg and 9 mg for 30 days each, then 25 mg from day 919.
Weight loss in each drug’s main trial (separate trials)ATTAIN-1, 3,127 adults without diabetes: 11.2% of body weight lost on the top dose (equal to the 17.2 mg tablet) versus 2.1% on placebo at 72 weeks67.OASIS 4, 307 adults without diabetes: 13.6% of body weight lost on 25 mg versus 2.2% on placebo at 64 weeks8.
Head to head in type 2 diabetes (ACHIEVE-3, 52 weeks)In 1,698 adults on metformin, HbA1c (a measure of average blood sugar) fell 1.71 and 1.91 percentage points on 12 and 36 mg, equal to the 9 and 17.2 mg tablets; weight fell 6.7% and 9.2% while people were taking it2427.HbA1c fell 1.23 and 1.47 percentage points on 7 and 14 mg, the Rybelsus doses; weight fell 3.7% and 5.3% while people were taking it245.
Side effects in ACHIEVE-3Digestive side effects in 59% and 58%; 9% and 10% stopped because of side effects; pulse up 3.7 and 4.7 beats a minute4.Digestive side effects in 37% and 45%; 4% and 5% stopped because of side effects; pulse up 1.0 and 1.5 beats a minute4.
Stopped because of side effects in the weight trials (label)8% over up to 72 weeks (6%, 9% and 10% on 5.5, 9 and 17.2 mg) versus 3% on placebo7.6.9% on 25 mg over up to 64 weeks versus 5.9% on placebo9.
Heart outcomesTested for safety, not benefit: in ACHIEVE-4 (2,749 adults with type 2 diabetes and heart or kidney disease), major heart and stroke events occurred in 4.2% versus 5.0% on insulin glargine, meeting the goal of showing it was not worse28. Lilly’s heart-outcomes trial in adults with obesity or overweight, ATTAIN-OUTCOMES, is ongoing29.Wegovy tablets are approved to lower the risk of heart attack, stroke and cardiovascular death in adults with heart disease and obesity or overweight9. In SELECT, which tested the weekly injection, these events occurred in 6.5% versus 8.0% on placebo30.
Taking other pillsIt slows stomach emptying, which may change how other pills are absorbed. The label advises people on birth control pills to switch to a non-oral method, or add a barrier method, for 30 days after starting and after each dose increase7.Other pills must wait at least 30 minutes. Levothyroxine (thyroid hormone) exposure rose 33% when taken with a semaglutide tablet, and the label advises monitoring other oral medicines9.
Self-pay prices announced at launch (US)$149 a month for the lowest dose (April 2026)31. Lilly’s 2025 agreement with the US government said other doses would cost up to $39932.$149 a month for the 1.5 mg starting dose (January 2026); Novo Nordisk said 4 mg would cost $199 after 15 April 2026 and the highest doses $29933.
Gray-market productsAs of 7 October 2026, Finnrick listed no orforglipron tests or vendors34. FDA’s Import Alert 66-80 lets it detain imported GLP-1 ingredients from makers not on its Green List35.74 of Finnrick’s latest 196 completed tests failed (37.8%) as of 30 September 202636. Fake semaglutide containing insulin has caused severe low blood sugar37.

Why is the Wegovy pill taken on an empty stomach?

Semaglutide is a peptide, and very little of a tablet is absorbed: about 1% to 2% of the dose reaches the bloodstream, against 89% of an injection9. Wegovy tablets contain an absorption enhancer, salcaprozate sodium (SNAC)9. In a study described in the label, healthy volunteers absorbed more semaglutide the longer they waited before eating, with waits of 15 to 120 minutes tested, while taking it with 50 mL or 120 mL of water made no clinically significant difference9. The label tells people to take the tablet in the morning on an empty stomach with up to 4 ounces of water, and to wait at least 30 minutes before eating, drinking or taking other pills9.

Orforglipron is a small molecule rather than a peptide, designed to be taken without food or water restrictions4. About 77% of a 0.8 mg dose reaches the bloodstream, food has no clinically relevant effect on how much is absorbed, and the label says to take it with or without food7.

Which causes more side effects?

ACHIEVE-3 is the only direct comparison. Digestive side effects affected 59% and 58% of people on the two orforglipron doses, against 37% and 45% on the two semaglutide doses, and 9% and 10% stopped orforglipron because of side effects, against 4% and 5% for semaglutide4. Pulse also rose more on orforglipron4. The trial was open-label, so participants knew which drug they were taking, and it tested semaglutide at the 7 and 14 mg Rybelsus doses, not the 25 mg Wegovy dose459.

The labels report the weight trials separately: 8% stopped Foundayo because of side effects, against 3% on placebo, and 6.9% stopped the 25 mg Wegovy pill, against 5.9% on placebo79. Both labels say side-effect rates from trials of different drugs cannot be directly compared79.

Both labels carry a boxed warning about thyroid C-cell tumors, and neither drug may be used by people with a personal or family history of medullary thyroid cancer or with MEN 2 (multiple endocrine neoplasia type 2)79. Semaglutide causes these tumors in rodents9. Orforglipron does not act in rats or mice and caused no tumors in them, but it does act on the human receptor, and its label says the human relevance of these rodent tumors has not been determined7.

Questions people ask

Is Foundayo better than the Wegovy pill?

ClinicalTrials.gov lists no trial that compares them as weight-loss treatments1. In ACHIEVE-3, a type 2 diabetes trial funded by Lilly, orforglipron lowered HbA1c more than 7 and 14 mg semaglutide tablets but caused more digestive side effects and more stopping4. In September 2026 Lilly announced an indirect comparison that combined ACHIEVE-3 with a separate trial of 25 mg semaglutide tablets in type 2 diabetes; assuming everyone stayed on treatment, it estimated 1.5% more weight loss and a 0.3-point larger fall in HbA1c on the top Foundayo dose3. In the same release, Lilly said head-to-head trials “ultimately provide the best comparison of two medicines”3.

Is orforglipron the same as semaglutide?

No. Both switch on the GLP-1 receptor, but semaglutide is a modified version of a natural peptide hormone and orforglipron is a small molecule that is not a peptide925. Semaglutide is sold as weekly injections and daily tablets; orforglipron only as a daily tablet, Foundayo79.

Do you have to take the Wegovy pill on an empty stomach?

Yes. Its label says to take it in the morning on an empty stomach with up to 4 ounces of water, and to wait at least 30 minutes before eating, drinking or taking other pills9. In a study, more semaglutide was absorbed the longer people waited before eating9. Foundayo has no such rule: its label says to take it with or without food7.

Does orforglipron cause more side effects than semaglutide?

In the one head-to-head trial, ACHIEVE-3, it did: digestive side effects affected 58% to 59% of people on orforglipron versus 37% to 45% on semaglutide tablets, and 9% to 10% versus 4% to 5% stopped because of side effects4. That trial was in type 2 diabetes and used the 7 and 14 mg semaglutide doses, not the 25 mg Wegovy pill49.

Can you switch from Wegovy injections to Foundayo?

The Foundayo label gives no directions for switching from another GLP-1 drug, and the Wegovy label covers switching only between its own injection and tablets79. In ATTAIN-MAINTAIN, adults whose weight had leveled off on semaglutide injections and who then took orforglipron kept an estimated 79.3% of the weight they had lost after 52 weeks, against 37.6% on placebo; the trial did not compare switching with staying on the injection27. Any switch is a decision for a prescriber.

Can you take orforglipron and semaglutide together?

Their labels advise against it. The Foundayo label says use with another GLP-1 receptor agonist is not recommended, and the Wegovy label says the same; each drug is one79.

How much does Foundayo cost compared with the Wegovy pill?

Both launched at $149 a month for the lowest dose for people paying for themselves in the US3133. Higher doses cost more: at launch Novo Nordisk listed $299 for the highest Wegovy pill doses, and Lilly’s 2025 agreement with the US government said Foundayo doses would cost up to $3993233. These are the prices the companies announced, not quotes from pharmacies.
