Semaglutide

Ozempic · Wegovy · Wegovy HD · Rybelsus · Ozempic tablets · Apo-Semaglutide (Canadian generic)

Semaglutide, the drug in Ozempic and Wegovy, cut weight and the risk of heart attack, stroke or heart-related death in large trials12. Unapproved “research” versions often fail tests3.

Quick answers

What is it?
A modified version of GLP-1, a gut hormone that helps control blood sugar and appetite613. Novo Nordisk sells it as a weekly injection or daily tablet: Ozempic and Rybelsus for type 2 diabetes, Wegovy for weight loss62223.
Does it work?
Yes, in large trials. People lost 14.9% of their weight on average over 68 weeks, versus 2.4% on placebo1. In overweight people with heart disease, it cut the risk of heart attack, stroke or heart-related death by 20%2. Two Alzheimer’s trials failed19.
Is it safe?
It has known risks. Nausea, diarrhea, vomiting and constipation are common and rise with dose167. It caused thyroid tumors in rats and mice; the risk in people is unknown6. The label also warns of an inflamed pancreas and gallbladder disease6.
Is it legal?
Yes, with a prescription, as Ozempic, Wegovy or Rybelsus46. US pharmacies may no longer make routine copies, because the shortage is over89. Selling “research” vials for human use is illegal in the US2043. Not banned in sport3435.
What is in the vials?
Often not as labeled. As of 30 September 2026, 74 of Finnrick’s latest 196 completed tests failed3. In a 2024 study, all 3 vials bought online held bacterial toxin, one at a raised level3839. Fake “semaglutide” with insulin put two people in a coma10.

What it is

Semaglutide is a modified form of GLP-1 (glucagon-like peptide-1), a gut hormone released after eating. Its 31-amino-acid chain is 94% identical to human GLP-1. Changes protect it from the enzyme DPP-4, and a fatty-acid chain binds it to albumin, the main protein in blood, so one dose lasts about a week613. It switches on the GLP-1 receptor. That raises insulin and lowers glucagon mainly when blood sugar is high, and it slows stomach emptying6. GLP-1 also helps regulate appetite and calorie intake, and in animal studies semaglutide reached and activated neurons in brain areas that control food intake6.

Novo Nordisk, which markets it, designed semaglutide as a once-weekly follow-up to liraglutide, a once-daily GLP-1 drug614. The 2015 discovery paper describes two amino-acid swaps (Aib at position 8, arginine at 34) and a fatty-acid chain on lysine 26, chosen to raise albumin binding and block breakdown14. FDA first approved it as Ozempic on 5 December 20174.

Benefits: what the research shows

Semaglutide is one of the most studied drugs in this catalog. In STEP 1 (1,961 adults with obesity or overweight and no diabetes), 2.4 mg a week plus lifestyle support cut body weight by 14.9% at 68 weeks versus 2.4% with placebo, and half the semaglutide group lost at least 15%1. In STEP UP (1,407 adults), 7.2 mg a week cut weight by 18.7% at 72 weeks versus 15.6% with 2.4 mg and 3.9% with placebo7. A daily 25 mg tablet cut weight by 13.6% versus 2.2% at 64 weeks in OASIS 4 (307 adults)16. In SELECT (17,604 adults with heart disease and overweight but no diabetes), heart attack, stroke or cardiovascular death occurred in 6.5% on semaglutide versus 8.0% on placebo over about 40 months, a 20% lower risk2. In FLOW (3,533 adults with type 2 diabetes and kidney disease), 1 mg a week cut the risk of kidney failure, major loss of kidney function, or death from kidney or heart causes by 24%17. In ESSENCE (interim analysis of 800 adults with MASH and liver fibrosis), MASH liver inflammation resolved without worse fibrosis in 62.9% on 2.4 mg versus 34.3% on placebo at 72 weeks18. Not every result is positive. In evoke and evoke+ (3,808 people with early Alzheimer’s disease), daily oral semaglutide did not slow decline on the main dementia rating scale over two years19. Side effects rise with dose. In STEP UP, digestive side effects affected 70.8% on 7.2 mg, 61.2% on 2.4 mg and 42.8% on placebo, and dysesthesia (unusual or unpleasant skin sensations) affected 22.9%, 6.0% and 0.5%7. In SELECT, 16.6% stopped treatment because of adverse events versus 8.2% on placebo2. All of this evidence comes from the approved, quality-controlled product. It says nothing about the content of unapproved vials sold online, which FDA calls products of unknown quality20.

Doses studied

Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.

In human studies

Approved labels: Wegovy injection starts at 0.25 mg once weekly and rises every 4 weeks (0.5, 1, 1.7 mg) to a maintenance dose of 2.4 mg (recommended) or 1.7 mg. Adults who tolerate 2.4 mg for at least 4 weeks may go up to 7.2 mg weekly if more weight loss is needed. Wegovy tablets start at 1.5 mg daily and rise every 30 days (4 mg, 9 mg) to 25 mg daily6. Ozempic starts at 0.25 mg weekly for 4 weeks, then 0.5 mg, up to a maximum of 2 mg weekly22. Rybelsus starts at 3 mg daily for 30 days, then 7 mg, then 14 mg if needed23. Trial doses: 2.4 mg weekly in STEP 1, SELECT and ESSENCE; 7.2 mg weekly in STEP UP; 1 mg weekly in FLOW; 25 mg daily by mouth in OASIS 4; up to 14 mg daily by mouth in evoke and evoke+12716171819.

In animal studies

Two-year cancer studies on the US label: mice received 0.3, 1 or 3 mg/kg/day (males) and 0.1, 0.3 or 1 mg/kg/day (females) under the skin; rats received 0.0025, 0.01, 0.025 or 0.1 mg/kg/day. Thyroid C-cell adenomas increased at every dose in both species, and C-cell carcinomas increased in male rats at 0.01 mg/kg/day and above6. In mini-pigs, plasma half-life after an intravenous dose was 46.1 hours14. These are animal doses, not human doses.

What users report Anecdotal · not validated · not a recommendation

“Microdosing” is described as 25% to 50% of the manufacturer’s recommended starting dose, or doses spaced further apart than the labeled schedule. A 2026 commentary says the practice gained traction during recent shortages through patient communities, telemedicine weight-loss programs and clinician anecdotes, and that high-quality evidence is essentially absent37.

Half-life
About 1 week6Elimination half-life in adults with overweight or obesity (Wegovy label). Semaglutide stays in the blood for about 5 to 7 weeks after the last dose. Bioavailability is 89% by injection under the skin and about 1% to 2% by tablet.
Routes
  • Subcutaneous injection once weekly (approved as Ozempic and Wegovy; human trial data)
  • Oral tablet once daily (approved as Rybelsus, Ozempic tablets and Wegovy tablets; human trial data)
  • Unapproved “research” vials sold for injection (no quality control; no trials of these products found)

Quality and the gray market

What testing finds

Independent tests show frequent problems with unapproved semaglutide. As of 30 September 2026, Finnrick had published 373 tests of semaglutide from 71 vendors. Of the latest 200, 122 passed, 74 failed and 4 were incomplete, a 37.8% fail rate among completed tests3. In a 2024 study, researchers ordered from 6 illegal online pharmacies. Only the 3 vial orders arrived. They held 28.6% to 38.7% more semaglutide than labeled, purity was 7.7% to 14.4% versus 99% claimed, and endotoxin, a bacterial toxin, was found in all three, at a raised level in one3839. The 3 Ozempic pen orders were never delivered, and the sellers demanded fake “customs” fees of $650 to $1,20039. On 14 September 2026 ABC News reported a TGA safety advisory: a product sold as retatrutide contained no retatrutide but held semaglutide at about eight times the level found in semaglutide products the TGA had assessed40. FDA warns that counterfeit Ozempic in the US may contain the wrong ingredients, or too little, too much or none of the active ingredient20.

How it is sold

Unapproved semaglutide is sold online as freeze-dried powder in vials38, often labeled “for research purposes” or “not for human consumption”. FDA says it has warned companies that sold such products directly to consumers with dosing instructions20. Code names do not change the legal status: FDA warning letters listed “GLP1-S” (31 March 2026) and “GLP-1 S (Semaglutide)” (24 August 2026) as unapproved new drugs4142. In letters dated 23 September 2026 and 24 September 2026, FDA said two websites selling “Semanat Injection (Semaglutide)” and “Semanext Injection” introduce “unapproved and misbranded injectable semaglutide drug products” into interstate commerce6162. DOJ has prosecuted sellers. On 23 July 2026 it announced a 21-month prison sentence for a Michigan man in a misbranding conspiracy case; the websites he sold through offered semaglutide and tirzepatide from China as “for research purposes only”43. On 15 April 2025 it announced that a Kentucky doctor had pleaded guilty to distributing misbranded semaglutide, some of it from vials labeled for lab research44. EMA and national EU regulators warned on 3 September 2025 of a sharp rise in illegal medicines sold as GLP-1 drugs45. Tester data suggest gray-market demand for semaglutide is modest next to newer drugs: by our count from Finnrick’s chart data, it drew 2.0% (434) of 21,253 test requests from May 2025 to May 2026, versus 34.6% for retatrutide46. On 30 September 2026, r/Semaglutide had 73,222 weekly visitors and r/Ozempic had 51,3114748. On a US Google Trends scale where the peak week for “peptides” is 100, “semaglutide” stayed at about 20 to 31 from January 2024 to September 202649. Legal cash prices have fallen. Wegovy cost $499 a month through NovoCare Pharmacy from 5 March 202550. The Wegovy pill launched on 5 January 2026 at $149 a month for the starting dose51. On 6 February 2026 the White House said TrumpRx prices for Ozempic and Wegovy injections average $350 a month and go as low as $19952. On 8 October 2026 NovoCare Pharmacy listed Wegovy injection at $349 a month for doses up to 2.4 mg and $399 for 7.2 mg, with $199 a month for new patients’ first two fills at 0.25 mg or 0.5 mg60. Finnrick showed gray-market prices of $1.00 to $28.33 per mg for its eight top-scored semaglutide sellers (tests through 20 September 2026)3. In a KFF poll published 14 November 2025, 17% of US adults who had taken a GLP-1 got it from an online provider or website and 9% from a medical spa53.

Side effects and risks

  • Digestive side effects are the most common (nausea, diarrhea, vomiting, constipation) and rise with dose67. In STEP 1, 4.5% stopped because of them versus 0.8% on placebo1.
  • Boxed warning: semaglutide causes thyroid C-cell tumors in rodents, and whether it does in humans is unknown. It is not for people with a personal or family history of medullary thyroid cancer or MEN 26.
  • Other label warnings: acute pancreatitis, gallbladder disease, low blood sugar when combined with insulin or drugs that boost insulin release, kidney injury from dehydration, severe digestive reactions, serious allergic reactions, diabetic eye complications, a faster heart rate, and inhaling stomach contents during anesthesia or deep sedation6. The label also says the four-dose Wegovy FlexTouch pen must never be shared between patients, even if the needle is changed, because of the risk of passing on blood-borne infections6.
  • Higher doses bring more side effects. At 7.2 mg, 22.9% reported dysesthesia versus 6.0% at 2.4 mg7.
  • Vision: EMA lists NAION, an optic-nerve injury that can cause vision loss, as a very rare side effect (up to 1 in 10,000 people). The 2026 US Wegovy, Ozempic and Rybelsus labels reviewed here do not list it6222326.
  • Mood: the Wegovy label dropped its warning on suicidal behavior and ideation in February 20266.
  • Dosing errors: FDA has received reports of adverse events, some requiring hospitalization, that may be linked to dosing errors with compounded injectable semaglutide, by patients measuring their own doses or by clinicians miscalculating them20.
  • Gray-market products can hold the wrong amount, bacterial endotoxin or a different drug, including insulin103840.
  • Legal risk: selling unapproved “research” semaglutide for human use is illegal in the US, and sellers have gone to prison2043.

Documented harms

  • 14 Sep 2026Australia: ABC News reported a TGA safety advisory about a user of a product sold as retatrutide who had uncontrollable vomiting, suffered a torn esophagus and needed hospital care. Testing found no retatrutide, but semaglutide at about eight times the level in semaglutide products the TGA had assessed.40
  • 29 Jun 2026Two people without diabetes or obesity, described in a Dutch medical journal, fell into a coma with severe low blood sugar after injecting “semaglutide” obtained without a prescription. It contained synthetic insulin, and they needed up to two days of glucose infusion.10
  • 17 Jun 2026A UT San Antonio study of US poison-center data reported that calls about GLP-1 receptor agonists rose from about 1,000 to 1,500 a year before 2021 to more than 8,000 in 2023, mostly from unintentional dosing or therapeutic errors.54
  • 31 May 2026FDA had received 990 adverse-event reports tied to compounded semaglutide. FDA says these events are likely underreported because state-licensed pharmacies that are not outsourcing facilities do not have to report them.20
  • 30 Jan 2026Case report: an 18-year-old woman developed euglycemic ketoacidosis (a dangerous acid build-up with normal blood sugar) 10 days after starting semaglutide bought online and raising the dose herself without supervision. She was discharged after 36 hours.55
  • 29 Jan 2026UK: the MHRA reported 1,296 Yellow Card reports of pancreatitis with GLP-1 and dual GIP/GLP-1 medicines from 2007 to October 2025 and strengthened its warnings.28

Questions people ask

Is semaglutide FDA approved?

Yes. It is approved as Ozempic and Rybelsus for type 2 diabetes and as Wegovy injection and tablets for weight management and heart risk reduction46. Semaglutide sold online as a “research” product is not an approved drug20.

Is “research” semaglutide safe?

FDA says these products are of unknown quality and may be harmful20. In Finnrick’s latest 200 tests, 37.8% of completed samples failed (as of 30 September 2026), and fake “semaglutide” that contained insulin put two people in a coma310.

Wegovy vs Ozempic: what’s the difference?

Both are once-weekly semaglutide injections from Novo Nordisk. Ozempic is labeled for type 2 diabetes at up to 2 mg a week. Wegovy is labeled for weight management, heart risk reduction and MASH at 2.4 mg a week, with up to 7.2 mg for weight loss in adults622.

Is semaglutide banned in sports?

No. WADA does not prohibit it in 2026 or 2027, but it is on WADA’s Monitoring Program and WADA has started a second study of its effects on athletes1236.

How long does semaglutide stay in your body?

Its half-life is about one week, so it stays in the blood for about 5 to 7 weeks after the last dose6.

What is still unknown

  • How much gray-market “semaglutide” is a salt form (sodium or acetate), which FDA says has no lawful basis for compounding, is unknown.
  • We found no controlled trial of “microdosing” regimens, so their effects on weight and safety are unknown.
  • EMA lists NAION as a very rare side effect, but the US labels reviewed (June to August 2026 versions) do not mention it. FDA’s current position was not checked.
  • Not verified, so left out: a January 2026 EU opinion for a product called Kayshild, Indian generic launches in March 2026, a June 2026 Canadian generic weight-loss launch, and whether Wegovy HD was the first GLP-1 approved under FDA’s national priority voucher pilot.
  • When Apotex’s tentatively approved US generic could be sold depends on patents and exclusivity, which were not reviewed.
  • Finnrick does not explain how its price-per-mg figures are derived, and its counts change as new tests are posted.
  • CDC data on emergency visits for semaglutide side effects (Ann Intern Med 2025) were not read.
  • UK marketing status, and rules outside the US, EU, UK, Canada and Australia, were not verified.
