Cagrilintide

AM833 (early code name) · NN0174-0833 (Novo Nordisk compound code) · CagriSema (fixed-dose combination with semaglutide) · Cagri (seller shorthand) · Cagrilinitide (misspelling seen on a seller’s site and in a DOJ release)

Cagrilintide is an experimental weekly injection from Novo Nordisk that caused large weight loss in trials when paired with semaglutide1. As of 30 September 2026, it is not approved, and vials sold online often fail tests23.

Quick answers

What is it?
A lab-made, long-acting version of amylin, a hormone that makes you feel full1011. Novo Nordisk developed it for weight loss and pairs it with semaglutide in an injection called CagriSema1216. Online sellers offer it labeled “research use only”3536.
Does it work?
Yes, in large trials. People taking it with semaglutide lost 20.4% of their weight on average over 68 weeks, versus 3.0% on placebo1. Alone, it worked less well than semaglutide alone6. The pair failed to match tirzepatide head to head7.
Is it safe?
Not fully known. Stomach side effects affected about 8 in 10 on CagriSema, versus about 4 in 10 on placebo1. Most were mild to moderate and short-lived1. Long-term safety and heart effects are unknown22.
Is it legal?
Not approved in the US or EU as of 30 September 202624. US pharmacies may not make it8. Novo expects an FDA decision on CagriSema in late 20265. Likely banned in sport while it stays unapproved9.
What is in the vials?
Almost half failed testing. From April 2025 to September 2026, 88 of 193 completed Finnrick tests failed checks of substance, purity or amount3. Those checks do not cover bacterial toxins333.

What it is

Cagrilintide is a lab-made, long-acting version of amylin, a hormone the pancreas releases along with insulin when you eat10. Natural amylin makes you feel full11. Amylin is hard to turn into a drug because it tends to clump into fibrils, and the older amylin drug pramlintide has to be injected three times a day12. Cagrilintide was designed to be stable and long-acting. Its half-life is about a week, so it is injected once weekly1213. It switches on amylin receptors and the closely related calcitonin receptor14. In mice lacking amylin receptors 1 and 3, it worked less well15. It is not a GLP-1 drug. Novo Nordisk combines it with the GLP-1 drug semaglutide in CagriSema to target complementary pathways16.

Novo Nordisk developed cagrilintide as a once-weekly amylin analogue for obesity1216. A 2021 paper described the structure-activity work that led to its selection for clinical development, as compound 2312. Another 2021 paper used the code name AM83314. An early trial combining it with semaglutide enrolled people between July 2018 and December 201913, and the phase 2 trial of cagrilintide alone enrolled people in 201911. Novo announced its FDA filing for CagriSema on 18 December 202516.

Benefits: what the research shows

Tier A rests on large, completed and published phase 3 trials of CagriSema, which pairs cagrilintide with semaglutide. Most of the evidence is therefore for the combination, not for cagrilintide alone. In REDEFINE 1 (3,417 adults without diabetes, 68 weeks), CagriSema cut body weight by 20.4% vs 3.0% on placebo1. In REDEFINE 2 (1,206 adults with type 2 diabetes, 68 weeks), the figures were 13.7% vs 3.4%19. In REIMAGINE 2 (2,713 adults with type 2 diabetes), CagriSema lowered HbA1c slightly more than semaglutide 2.4 mg alone (1.91 vs 1.75 points), and the authors called its safety profile consistent with the GLP-1 drug class20. It did not match the strongest rival. In the open-label REDEFINE 4 trial (809 people, 84 weeks), CagriSema failed to show it was at least as good as tirzepatide 15 mg (23.0% vs 25.5% weight loss if everyone stayed on treatment), per Novo’s topline release of 23 February 20267. Novo reported on 21 September 2026 that a lower CagriSema dose (1.0 mg/1.0 mg) beat tirzepatide 5 mg on weight loss at 60 weeks in people with type 2 diabetes (12.4% vs 9.1%, REIMAGINE 5)21. Effects on heart attacks, strokes and deaths are not yet known. The REDEFINE 3 outcomes trial (7,101 people) has an estimated primary completion date of 1 September 202722. Cagrilintide alone works, but less well than semaglutide. In a 26-week phase 2 trial of 706 adults, weekly doses of 0.3 to 4.5 mg cut weight by 6.0% to 10.8% vs 3.0% on placebo, and 4.5 mg slightly beat daily liraglutide 3.0 mg (10.8% vs 9.0%)11. In REDEFINE 1, if everyone had stayed on treatment, weight fell 11.8% with cagrilintide alone, 16.1% with semaglutide alone and 22.7% with both6. In REIMAGINE 2, on the same basis, cagrilintide alone cut weight by 8.4% and HbA1c by 0.80 points in people with type 2 diabetes10. Two phase 3 trials of cagrilintide alone, RENEW 1 and RENEW 2, started on 5 November 2025, with primary completion estimated for May 20272324. A dedicated heart-rhythm study in healthy volunteers found no clinically relevant QT prolongation at doses up to 4.5 mg25. Animal studies hint at how it works. In rats, about a third of CagriSema’s weight loss came from blunting the usual drop in energy use that comes with weight loss26. In mice lacking amylin receptors 1 and 3, cagrilintide lost potency15.

Quality and the gray market

What testing finds

Finnrick, a US testing service, lists 194 cagrilintide tests from 43 vendors between 20 April 2025 and 14 September 20263. Under its default Purity focus standard (correct substance, purity of at least 99.5% and an amount within 35% of the label claim), 88 samples failed, 105 passed and 1 was incomplete. That means 46% of completed tests failed333. By our count from the public list, the fail share has stayed between 43% and 47% in every half-year since testing began3. The public page does not show each sample’s measured purity or dose without a login, and endotoxin is not graded under this standard333. Finnrick says it sources samples from vendors, and it also offers free US mail-in testing, so results may not represent the whole market333. FDA’s recall database had no cagrilintide entries when we checked on 1 October 202634.

How it is sold

Cagrilintide is sold online as injectable products labeled for research use, sometimes as ‘Cagri’ and in blends. FDA warning letters to online sellers dated 10 December 2024 and 31 March 2026 called their cagrilintide products unapproved new drugs, despite ‘RESEARCH USE ONLY’ and ‘not for human consumption’ labels3536. One seller’s May 2024 Facebook post promised ‘Enhanced Weight Loss’ from ‘Cagrilintide and Semaglutide’35. Another claimed that ‘weight loss of 25% or more was achieved in 40.4% of patients receiving Cagri compared to 16.2% with semaglutide alone’36. Yet in REDEFINE 1, cagrilintide alone produced less weight loss than semaglutide alone6. On 30 September 2026, one seller’s site code hid ‘cagrilinitide tre’ and ‘semacargritre blend’ from logged-out visitors37. A Utah physician was indicted on 1 April 2026 for allegedly receiving misbranded drugs from China, including ‘Cagrilinitide [sic]’; prosecutors say the drugs reached ‘over 200 trusting patients’. These are allegations38. Demand is modest. Cagrilintide made up 485 of 21,253 Finnrick test requests (2.3%) from May 2025 to May 2026, rising from 5 to 49 a month39. US Google searches for it never rose above 1 on a scale where ‘peptides’ peaks at 100 (January 2024 to September 2026)40. We found no verified price data.

Side effects and risks

  • Stomach and gut side effects are common. In the 26-week phase 2 trial of cagrilintide alone, 41% to 63% had GI events vs 32% on placebo, mainly nausea, and injection-site reactions were also frequent11. With CagriSema in REDEFINE 1, 55% had nausea, 30.7% constipation and 26.1% vomiting, vs 12.6%, 11.6% and 4.1% on placebo16.
  • Side effects made 5.9% of CagriSema users stop treatment in REDEFINE 1 (3.5% on placebo) and 8.4% in REDEFINE 2 (3% on placebo)16.
  • Long-term safety and effects on heart disease are not known. The 7,101-person REDEFINE 3 outcomes trial has an estimated primary completion date of 1 September 202722.
  • Gray-market products are unapproved and of uncertain content: 88 of 193 completed Finnrick tests failed its quality standard3. FDA warns that injectable products bypass some of the body’s key defenses against toxins and microorganisms36.
  • Stacks with other weight-loss drugs lack human data. Cagrilintide plus retatrutide has been tested in rats41, and none of the 44 cagrilintide studies on ClinicalTrials.gov (checked 1 October 2026) gives it with retatrutide or tirzepatide42.
  • Anti-doping: likely prohibited at all times under WADA’s S0 class while it stays unapproved9.

Documented harms

  • 27 Mar 2026FDA’s adverse event reporting system (FAERS) received a spontaneous report from a pharmacist about a 33-year-old woman hospitalized with hives, itching and shortness of breath. Cagrilintide was the only drug listed. It is one of 4 FAERS reports naming cagrilintide in openFDA data through 30 July 2026. FAERS reports are unverified and do not prove the drug caused the event43.

Questions people ask

Is cagrilintide FDA approved?

No. As of 30 September 2026, no product containing cagrilintide is FDA-approved28. Novo Nordisk filed for approval of CagriSema (cagrilintide plus semaglutide) in December 2025 and expects a decision in Q4 2026516.

What is CagriSema?

CagriSema is Novo Nordisk’s investigational once-weekly injection that combines cagrilintide 2.4 mg with semaglutide 2.4 mg16. In the 68-week REDEFINE 1 trial it cut body weight by 20.4% vs 3.0% on placebo1.

Cagrilintide vs semaglutide: which works better?

Semaglutide alone caused more weight loss than cagrilintide alone in REDEFINE 1 (16.1% vs 11.8% at 68 weeks if everyone stayed on treatment), and the two together did best at 22.7%6. In people with type 2 diabetes, cagrilintide alone caused less weight loss than semaglutide 2.4 mg (8.4% vs 10.2%) but fewer stomach or gut side effects (39.5% vs 53.9%)10.

Can cagrilintide be compounded?

No. FDA says cagrilintide cannot be used in compounding under federal law and is not a component of any FDA-approved drug8. It is also in none of the categories on FDA’s 503A bulk drug substance list27.

Is CagriSema better than tirzepatide?

Not at the top doses. In the 84-week REDEFINE 4 trial, CagriSema 2.4 mg/2.4 mg failed to show it was at least as good as tirzepatide 15 mg (23.0% vs 25.5% weight loss if everyone stayed on treatment)7. Novo reported that a lower CagriSema dose beat tirzepatide 5 mg on weight loss in people with type 2 diabetes (12.4% vs 9.1% at 60 weeks)21.

Is cagrilintide banned in sport?

WADA does not name it, but its S0 rule bans at all times any drug with no current government approval for human use, and cagrilintide had none in the US or EU as of 30 September 2026249. The 2027 List, in force from 1 January 2027, adds ‘peptides’ to the S0 examples30.

What is still unknown

  • Will FDA approve CagriSema? Novo expects a decision in Q4 2026.
  • How well does cagrilintide work alone in dedicated phase 3 trials? RENEW 1 and 2 have estimated primary completion in May 2027.
  • Does CagriSema reduce heart attacks, strokes or deaths? REDEFINE 3 has estimated primary completion on 1 September 2027.
  • Full results of REDEFINE 4 and REIMAGINE 5 were not in PubMed as of 1 October 2026; the figures here come from Novo press releases.
  • What gray-market cagrilintide contains beyond identity, purity and amount: sterility, endotoxin and impurity data are not on Finnrick’s public page.
  • Regulatory status in Canada, the UK and Australia was not verified from regulator records.
  • No verified price data and no citable community dosing page were found.
