Cagrilintide dosage: what studies actually used
The short answer
No dose of cagrilintide is approved: when we checked on 7 October 2026, no product containing it was approved in the US or the EU12. Novo Nordisk’s US application, filed in December 2025, is for CagriSema, a once-weekly combination of cagrilintide 2.4 mg and semaglutide 2.4 mg18. The table lists first the trials that gave cagrilintide alone, at 0.3 to 4.5 mg once a week, then the trials that gave it only with semaglutide, as CagriSema, largest first within each group1920. This page reports those doses and is not dosing advice.
Not dosing advice. This page reports doses that studies and approved labels used, with their sources. It does not recommend a dose, and we do not give mixing, injection or unit-conversion instructions. Gray-market vials often do not contain what their label says. How we report doses.

HbA1c fell 0.9 percentage points on cagrilintide alone, 1.8 on semaglutide and 2.2 on CagriSema, and weight fell 8.1%, 5.1% and 15.6%3. Adverse events were reported by 80% on cagrilintide alone, 71% on semaglutide and 68% on CagriSema; mild or moderate digestive ones were the most common3. |
| Kidney and liver studies (Nielsen 2026), cagrilintide alone, single-dose studies | Adults with normal kidney or liver function, or with mild, moderate or severe impairment | – | One dose: 0.6 mg in the kidney study, 0.9 mg in the liver study Injection under the skin · Single dose, measured to day 36 or day 39 |
The kidney study had 33 people and the liver study 32; cagrilintide levels in the blood were similar across the groups, and no serious adverse events were reported4. |
| REDEFINE 2 (Davies 2025), CagriSema, randomized trial | Adults with type 2 diabetes and a BMI of 27 or more | 1,206 | CagriSema 2.4 mg/2.4 mg once a week, or placebo Injection under the skin · 68 weeks |
Counting everyone regardless of adherence, weight fell 13.7% versus 3.4% on placebo, and 73.5% reached an HbA1c of 6.5% or less, versus 15.9%25. Digestive side effects affected 72.5% versus 34.4%, and 8.4% stopped because of side effects, versus 3%1825. |
| REIMAGINE 4 (Novo half-year report 2026), CagriSema, open-label randomized trial | Adults with type 2 diabetes and a BMI of 30 or more, on metformin with or without an SGLT2 inhibitor | 1,024 | CagriSema 2.4 mg/2.4 mg once a week after a 16-week escalation, versus tirzepatide 15 mg after a 20-week escalation Injection under the skin · 68 weeks |
If everyone had stayed on treatment, weight fell 15.2% versus 15.8% on tirzepatide 15 mg, and HbA1c fell 1.9 versus 2.2 percentage points5. Novo says CagriSema was shown to be no worse than tirzepatide for weight loss, but not for HbA1c5. Novo described the trial in February 2026 as having 1,000 adults6. The registry entry that matches that description, though it does not use the trial’s name, lists 1,024 people7. |
| REIMAGINE 5 (Novo press release 2026), CagriSema, open-label randomized trial | Adults with type 2 diabetes on metformin, an SGLT2 inhibitor or both | 1,023 | CagriSema 1.0 mg/1.0 mg once a week after an 8-week escalation, versus tirzepatide 5 mg once a week Injection under the skin · 60 weeks |
If everyone had stayed on treatment, weight fell 12.4% on CagriSema versus 9.1% on tirzepatide 5 mg at week 6026. HbA1c fell 1.71 versus 1.67 percentage points, which Novo says showed CagriSema was no worse than tirzepatide26. The registry entry lists 1,023 people8. |
| REDEFINE 4 (Novo press release 2026), CagriSema, open-label randomized trial | People with obesity and at least one related condition | 809 | CagriSema 2.4 mg/2.4 mg once a week after a 16-week escalation, versus tirzepatide 15 mg after a 20-week escalation Injection under the skin · 84 weeks |
If everyone had stayed on treatment, weight fell 23.0% versus 25.5% on tirzepatide, and counting everyone regardless of adherence, 20.2% versus 23.6%27. The trial missed its main goal of showing CagriSema was no worse than tirzepatide for weight loss27. |
| REDEFINE 5 (Yamauchi 2026), CagriSema, randomized trial | Adults in Japan and Taiwan with a BMI of 27 or more and at least two obesity-related conditions, or 35 or more and at least one, with or without type 2 diabetes | 331 | CagriSema 2.4 mg/2.4 mg or semaglutide 2.4 mg once a week, each reached in 4-week steps from 0.25 mg over 16 weeks Injection under the skin · 68 weeks |
If everyone had taken the treatment as intended, weight fell 18.4% on CagriSema versus 11.9% on semaglutide 2.4 mg9. Adverse events were reported by 87% versus 84%, most often digestive (53% versus 51%)9. |
| REDEFINE 9 (Novo press release 2026), CagriSema, placebo-controlled trial | Adults with overweight or obesity | 300 | CagriSema 1.0 mg/1.0 mg or 1.7 mg/1.7 mg once a week, or placebo Injection under the skin · 68 weeks |
If everyone had stayed on treatment, weight fell 21.0% on 1.0 mg/1.0 mg versus 2.0% on placebo in this 300-person trial2628. Novo’s half-year report of 4 August 2026 says 1.7 mg/1.7 mg also beat placebo, without giving a figure, and that detailed results would be presented at a scientific conference later in 20265. |
| REIMAGINE 3 (Rosenstock 2026), CagriSema, randomized trial | Adults with type 2 diabetes on once-daily long-acting (basal) insulin, with or without metformin | 274 | CagriSema 2.4 mg/2.4 mg or 1.0 mg/1.0 mg once a week, or placebo Injection under the skin · 40 weeks |
If everyone had stayed on treatment, HbA1c fell 2.33 and 2.10 percentage points on the two doses, versus 0.66 on placebo, and weight fell 12.0% and 10.4%, versus a 1.1% gain1022. No severe hypoglycemia (dangerously low blood sugar) was reported10. |
| REIMAGINE 1 (Aroda 2026), CagriSema, randomized trial | Adults with type 2 diabetes not controlled by diet and exercise alone | 189 | CagriSema 2.4 mg/2.4 mg or 1.0 mg/1.0 mg once a week, or placebo Injection under the skin · 40 weeks |
If everyone had stayed on treatment, HbA1c fell 1.8 and 1.5 percentage points on the two doses, versus 0.1 on placebo, and weight fell 13.8% and 11.8%, versus 1.4%1122. Digestive side effects affected 53% and 44%, versus 20% on placebo22. |

At week 20, weight fell 15.7% on cagrilintide 1.2 mg and 17.1% on 2.4 mg, versus 9.8% on placebo with semaglutide, and 15.4% on 4.5 mg versus 8.0% on its matched placebo29. Of 566 adverse events, 207 were digestive, and most were mild to moderate29. |
Studies, animals and users: kept apart
Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.
In human studies
Alone: 0.3 to 4.5 mg once weekly for 26 weeks in the phase 2 trial19; 2.4 mg once weekly in the cagrilintide-only arms of REDEFINE 1 and REIMAGINE 22023; escalated to 4.5 mg once weekly in healthy volunteers in the heart-rhythm study24. With semaglutide: CagriSema 2.4 mg/2.4 mg once weekly, reached after a 16-week dose escalation, in REDEFINE 130; CagriSema 2.4 mg/2.4 mg or 1.0 mg/1.0 mg in the REIMAGINE diabetes trials2226. Every trial cited here gave it by injection under the skin (subcutaneous).
In animal studies
Diet-induced obese rats: cagrilintide 2 or 10 nmol/kg alone, or 2 nmol/kg with semaglutide 2 nmol/kg (CagriSema, after a 2-day ramp-up), injected under the skin once daily for 21 days31. Diet-induced obese mice: 3 nmol/kg under the skin once daily for 3 weeks32. These are rodent research doses, not human doses.

Half-life and how it is taken
- Half-life
- 159 to 195 hours (about 6.6 to 8.1 days)29Phase 1b trial in otherwise healthy adults with overweight or obesity; cagrilintide 0.16 to 4.5 mg once weekly given with semaglutide 2.4 mg; median time to peak level 24 to 72 hours.
- Routes
- Subcutaneous injection, once weekly (human data from phase 1 to phase 3 trials)
- Subcutaneous injection, once daily (rats and mice only)
How the trials raised the dose
In the phase 2 trial of cagrilintide alone, the 0.3 mg and 0.6 mg groups took their dose from the start, while the higher-dose groups began at 0.6 mg and stepped up every 2 weeks12. On that schedule, the 1.2 mg group reached its dose at week 2, the 2.4 mg group at week 4 and the 4.5 mg group at week 6, and each then stayed on its dose to week 2612. The heart-rhythm study in healthy volunteers also began at 0.6 mg and raised the dose every 2 weeks, over 8 weeks, to 4.5 mg13.
The phase 2 diabetes trial and REDEFINE 5 used 4-week steps1415. In the phase 2 diabetes trial, every group, including the one on cagrilintide alone, took 0.25 mg in weeks 1 to 4, 0.5 mg in weeks 5 to 8, 1.0 mg in weeks 9 to 12 and 1.7 mg in weeks 13 to 16, then 2.4 mg from week 17 to week 3214. REDEFINE 5 used the same four steps over 16 weeks in both its groups, before 52 weeks on CagriSema 2.4 mg/2.4 mg or semaglutide 2.4 mg15.
REDEFINE 1 gave all three active groups, cagrilintide alone included, a 16-week escalation followed by 52 weeks at the full dose30. REDEFINE 4 and REIMAGINE 4 used the same 16 weeks for CagriSema, while their tirzepatide groups took 20 weeks to reach 15 mg716. REIMAGINE 5, which aimed for CagriSema 1.0 mg/1.0 mg, raised the dose every 4 weeks over an 8-week escalation and then kept it for 52 weeks826. The phase 1b trial raised cagrilintide and semaglutide together every 4 weeks over 16 weeks, then kept everyone on the target dose for 4 weeks29.
What a higher dose of cagrilintide alone changed
In the phase 2 trial, average weight loss grew with each dose12. The registry’s posted results, which use everyone’s weights for as long as they stayed in the trial, whether or not they kept taking the drug, show 6.1% on 0.3 mg, 6.9% on 0.6 mg, 8.5% on 1.2 mg, 9.5% on 2.4 mg and 10.8% on 4.5 mg at 26 weeks, versus 8.5% on liraglutide and 3.0% on placebo12. The trial’s statistical model estimated that 15.28% of people on 0.3 mg and 53.49% on 4.5 mg lost at least 10% of their weight, versus 10.44% on placebo12.
Nausea was the main digestive side effect19. It was reported by 20 of 101 people on 0.3 mg, 27 of 100 on 0.6 mg, 37 of 102 on 1.2 mg, 32 of 102 on 2.4 mg and 47 of 101 on 4.5 mg, versus 18 of 101 on placebo12.
Cagrilintide alone or with semaglutide
At 2.4 mg a week, cagrilintide alone lowered weight less than semaglutide alone in both phase 3 trials that have reported results for it, and the two together lowered it most2122. In the smaller phase 2 diabetes trial, cagrilintide alone lowered weight more than semaglutide alone (8.1% versus 5.1% at 32 weeks), though semaglutide lowered HbA1c more3.
In REIMAGINE 2, cagrilintide alone caused fewer digestive side effects than semaglutide or the combination22. They affected 39.5% of people on cagrilintide alone, 53.9% on semaglutide 2.4 mg and 67.2% on CagriSema 2.4 mg/2.4 mg, and 4.6%, 6.6% and 8.5% stopped treatment because of side effects22.
Semaglutide, the other half of CagriSema, is a different molecule, already approved as Wegovy, and its doses do not carry over to cagrilintide; they are set out on the semaglutide dose page1833. Pramlintide, an amylin drug already sold for diabetes alongside insulin, is a different molecule too: it is short-acting and needs three injections a day, so its doses do not apply to cagrilintide either34.
What an approval would and would not cover
When we checked on 7 October 2026, FDA’s approvals database listed no product containing cagrilintide or named CagriSema, and CagriSema was not among FDA’s novel drug approvals of 2026, a list current to 28 September 2026117. EMA’s medicines data for 7 October 2026 list no medicine containing cagrilintide, and none was on EMA’s September 2026 list of medicines under evaluation235.
Novo Nordisk filed for US approval of CagriSema in December 2025, and says it also filed in Canada that month1828. On 30 September 2026 it still called CagriSema investigational, meaning not yet approved, and expected an FDA decision in the last quarter of 202628.
The US application covers one product: CagriSema, a fixed-dose combination of cagrilintide 2.4 mg and semaglutide 2.4 mg injected once a week, to reduce excess body weight and keep it off long term in adults with obesity, or overweight with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity18. An approval would set doses for that combination, not for cagrilintide on its own18.
Cagrilintide alone has two phase 3 trials of its own, RENEW 1 and RENEW 2, which compare it with placebo once a week for 64 weeks in people with overweight or obesity, with type 2 diabetes as well in RENEW 23637. Their registry entries, last updated in August and September 2026, do not state the dose, say that doctors may not yet prescribe cagrilintide, and expect to finish collecting their main results in May 20273637.
Novo also started a phase 3 trial in 2026 that compares CagriSema 2.4 mg/7.2 mg with CagriSema 2.4 mg/2.4 mg and with semaglutide 7.2 mg in people with obesity, and in its report of 4 August 2026 expected results in the first half of 2028528.
Why gray-market doses are unreliable
FDA says federal law does not allow cagrilintide to be used in compounding, that it is not a component of any FDA-approved drug, and that it has not been found safe and effective for any condition38.
As of 1 October 2026, Finnrick, a US testing service, listed 194 cagrilintide tests from 43 vendors between 20 April 2025 and 14 September 2026: 88 failed, 105 passed and 1 was incomplete39. Its pass mark already allows the amount in a sample to differ from the label by up to 35%, and its public page did not show each sample’s measured amount without a login3940. So the amount in a gray-market vial labeled with a trial dose may differ from its label, and Finnrick’s public results did not show by how much3940.
Questions people ask
How much cagrilintide should I take?
No dose is established: when we checked on 7 October 2026, no product containing cagrilintide was approved in the US or the EU12. Trials gave it on its own at 0.3 to 4.5 mg once a week, and the phase 3 trials that have reported results for it alone used 2.4 mg192022. Ask your prescriber about approved options.
How much cagrilintide is in CagriSema?
The CagriSema filed for US approval combines 2.4 mg of cagrilintide with 2.4 mg of semaglutide, taken once a week18. Trials also tested combinations with 1.0 mg or 1.7 mg of each, and a trial started in 2026 tests 2.4 mg of cagrilintide with 7.2 mg of semaglutide52226.
What is the highest dose of cagrilintide studied?
4.5 mg once a week, the top dose in the phase 2 trial, the heart-rhythm study and the phase 1b trial with semaglutide192429. The two largest trials, REDEFINE 1 and REIMAGINE 2, used 2.4 mg, the amount in the CagriSema filed for US approval182022.
How long did trials take to reach the full cagrilintide dose?
In the phase 2 trial of cagrilintide alone, the 4.5 mg group reached its dose at week 6, stepping up every 2 weeks12. REIMAGINE 5 raised CagriSema every 4 weeks to reach 1.0 mg/1.0 mg after 8 weeks, and REDEFINE 5 used 4-week steps to reach 2.4 mg/2.4 mg after 16 weeks81526.
Will cagrilintide be approved on its own?
Novo Nordisk’s applications, filed in the US and Canada in December 2025, are for CagriSema, the combination with semaglutide, not for cagrilintide alone1828. Cagrilintide alone is being tested in two phase 3 trials, RENEW 1 and RENEW 2, whose registry entries, last updated in August and September 2026, expect to finish collecting their main results in May 20273637.
