Orforglipron

Foundayo (brand name; US and UK) · LY3502970 (Eli Lilly development code) · OWL833 (Chugai’s code before it licensed the drug to Lilly)

Orforglipron (Foundayo) is Eli Lilly’s daily weight-loss pill, FDA-approved in April 20261. It is not a peptide2. On the top dose, adults lost 11.2% of their weight on average over 72 weeks, versus 2.1% on placebo2.

Quick answers

What is it?
A daily pill from Eli Lilly, sold as Foundayo3. It acts like GLP-1, a hormone released after eating that helps control appetite34. It is a small drug molecule, not a peptide, but works through the same hormone signal as semaglutide25.
Does it work?
Yes, in large trials. On the top dose, adults without diabetes lost 11.2% of their weight over 72 weeks, versus 2.1% on placebo2. Adults with type 2 diabetes lost 9.6%, versus 2.5%6. In diabetes, it lowered blood sugar more than a semaglutide pill5.
Is it safe?
It has known risks. Nausea, constipation, diarrhea and vomiting are common3. The label warns of possible thyroid tumors, an inflamed pancreas and gallbladder disease3. In a 2-year diabetes trial, serious heart and stroke events were no more common than on insulin7.
Is it legal?
Yes, with a prescription, as Foundayo, approved in the US on 1 April 20261. The UK approved it on 10 August 20264. Whether US pharmacies may make it is unclear8. It is not named on sport’s banned list, and as an approved drug it is likely allowed910.
What is in the vials?
Foundayo is a tablet, not a vial3. As of 7 October 2026, Finnrick, a testing service, listed no sellers and no tests of unbranded orforglipron11.

What it is

Orforglipron is a lab-made drug taken as a once-daily tablet. It binds to and switches on the human GLP-1 receptor, the target of GLP-1, a natural hormone that helps regulate appetite and how much people eat3. These receptors sit in brain areas that control appetite, and in animal studies orforglipron reached and activated nerve cells there3. It lowers food intake, most likely by reducing appetite, and it slows stomach emptying, an effect that is largest after the first dose and fades over time; more of the weight lost is fat than lean tissue3. In lab tests it binds the human receptor tightly and selectively, and occupying a small share of receptors is enough for a full effect14. Unlike semaglutide and the other peptide drugs in this class, which are chains of amino acids, orforglipron is a small nonpeptide molecule214. It is covered on this site because it acts on the same receptor as semaglutide and tirzepatide and has been tested as a follow-on to their injections515. It is absorbed well by mouth: about 77% of a 0.8 mg dose reaches the bloodstream, and food has no clinically relevant effect on how much is absorbed3. Semaglutide tablets, by contrast, must be taken on an empty stomach with no more than 4 ounces of water16. Orforglipron is not active in rats, mice or rabbits, so the standard animal studies could not test for the thyroid tumors that other GLP-1 drugs cause in rats; the label assumes it would cause them if it were active in rats3.

Orforglipron was discovered by Chugai Pharmaceutical, a Tokyo-based drug company, which called it OWL8331718. On 27 September 2018 Chugai licensed worldwide rights to Eli Lilly for an upfront payment of $50 million, describing it as an oral non-peptide GLP-1 drug ready for first human trials in type 2 diabetes17. Lilly developed it under the code LY350297019. A Phase 2 trial in 272 adults with obesity or overweight, published in 2023, found weight losses of 9.4% to 14.7% at 36 weeks versus 2.3% on placebo20. Lilly then ran Phase 3 trials named ATTAIN, for weight, and ACHIEVE, for type 2 diabetes221. FDA received the new drug application on 20 January 2026 and approved it as Foundayo on 1 April 20261.

Benefits: what the research shows

Orforglipron has been tested in large, controlled trials, all funded by Lilly256. In ATTAIN-1 (3,127 adults with obesity and no diabetes), daily 6, 12 or 36 mg cut weight by 7.5%, 8.4% and 11.2% at 72 weeks versus 2.1% on placebo, by the treatment-regimen estimand, which counts people who stopped the drug. On 36 mg, 54.6% lost at least 10% of their weight and 18.4% at least 20%, versus 12.9% and 2.8% on placebo2. ATTAIN-1’s registry record lists it as active, not recruiting, with completion expected in October 202719. In ATTAIN-2 (1,613 adults with obesity or overweight and type 2 diabetes), the same doses cut weight by 5.1%, 7.0% and 9.6% versus 2.5%6. These trials used an earlier capsule formulation: 6, 12 and 36 mg correspond to the 5.5, 9 and 17.2 mg Foundayo tablets, and the label reports the results under the tablet doses315. In ATTAIN-MAINTAIN, 376 adults who had lost weight on tirzepatide or semaglutide injections were switched to orforglipron or placebo for 52 weeks. Among those whose weight had leveled off before the switch, people on orforglipron kept an estimated 74.7% (after tirzepatide) and 79.3% (after semaglutide) of the weight they had lost, versus 49.2% and 37.6% on placebo15. For comparison, the semaglutide pill at 25 mg, the Wegovy tablet dose, cut weight by 13.6% versus 2.2% at 64 weeks in a separate trial, OASIS 4; the trials differ, so the numbers cannot be compared directly23. In type 2 diabetes, the main measure is HbA1c, a blood test that reflects average blood sugar. In ACHIEVE-1 (559 adults with early type 2 diabetes), 40 weeks of 3, 12 or 36 mg lowered HbA1c by 1.24 to 1.48 percentage points versus 0.41 on placebo, and weight by 4.5% to 7.6% versus 1.7%21. It lowered HbA1c more than dapagliflozin, another diabetes pill, in ACHIEVE-2 (962 adults), and more than placebo when added to insulin glargine in ACHIEVE-5 (546 adults)2425. In the open-label ACHIEVE-3 (1,698 adults on metformin), 12 and 36 mg lowered HbA1c by 1.71 and 1.91 points over 52 weeks, more than the 1.23 and 1.47 points on 7 and 14 mg of oral semaglutide, the Rybelsus doses526. But gut side effects (59% and 58% vs 37% and 45%), stopping because of side effects (9% and 10% vs 4% and 5%) and the rise in pulse were all greater on orforglipron5. In ACHIEVE-4 (2,749 adults with type 2 diabetes and heart or kidney disease, median follow-up 2 years), major heart and stroke events occurred in 4.2% on orforglipron versus 5.0% on insulin glargine, meeting the trial’s goal of showing it is not worse7. In a 52-week safety trial in Japan, 5% to 14% stopped because of side effects, most often on the top dose27. A pooled analysis of seven Phase 3 trials found no cases that met criteria for drug-induced liver injury28. Across trials, gut side effects such as nausea were the most common, mostly mild to moderate and mostly while the dose was being raised2621. Phase 3 trials in sleep apnea, high blood pressure, stress urinary incontinence, osteoarthritis pain and peripheral artery disease were under way in 202629.

Doses studied

Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.

In human studies

Foundayo label (weight management): start at 0.8 mg by mouth once daily; after at least 30 days, increase to 2.5 mg, and after at least 30 days on 2.5 mg, to 5.5 mg. The dose may then be raised to 9, 14.5 or 17.2 mg once daily, each step after at least 30 days on the current dose, based on response and tolerability. The maximum is 17.2 mg once daily, or 9 mg with a strong CYP3A4 inhibitor3. The label says to take it with or without food and to swallow the tablet whole3. The Phase 3 trials used an earlier capsule formulation: 6, 12 or 36 mg daily for 72 weeks in the weight trials26 and doses of 3 to 36 mg daily for 40 to 52 weeks in the diabetes trials5212425. Capsule doses of 1, 3, 6, 12, 24 and 36 mg correspond to tablets of 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg15, and the label reports the weight trials under the tablet doses3. The Phase 2 weight trial used 12, 24, 36 or 45 mg daily for 36 weeks20.

In animal studies

From the Foundayo label: in a 2-year rat study, oral doses of 5, 30 and 200 mg/kg/day (about 3, 9 and 26 times human exposure at the maximum dose) caused no tumors, and a 26-week study in Tg.RasH2 transgenic mice at the same doses also found none. Orforglipron is not active in rats or mice, so these studies could not show GLP-1-driven thyroid tumors3. Rat fertility was unaffected at up to 200 mg/kg/day3. In pregnant monkeys, 0.15 and 0.7 mg/kg/day (exposure below the human maximum) had no effect on the fetus; higher doses could not be given because of effects on the mothers’ body weight3. In a rabbit dose-finding study at 2, 20 and 200 mg/kg/day, the top dose (about 14 times human exposure) lowered fetal and placental weights, and 2 fetuses from 2 mothers had external malformations; a definitive rabbit study at 4, 40 and 200 mg/kg/day (up to about 6 times human exposure) found no effects on the fetus. Orforglipron is not active in rabbits either3. These are animal safety-study doses, not human doses.

What users report

Half-life
About 29 to 49 hours3After an oral dose (Foundayo label, clinical pharmacology section). Blood levels peak 4 to 8 hours after a dose, and steady levels are reached after about 1 week of daily dosing. About 77% of a 0.8 mg dose reaches the bloodstream, food has no clinically relevant effect on absorption, and more than 99% is bound to blood proteins. It is broken down mainly by the liver enzyme CYP3A4.
Routes

Quality and the gray market

What testing finds

No independent test results were found. As of 7 October 2026, Finnrick’s orforglipron page listed 0 vendors and 0 tests11. FDA’s Import Alert 66-80 lists orforglipron ingredient from three manufacturers on its Green List of makers exempt from automatic detention; when FDA evaluated 48 GLP-1 ingredient manufacturing sites, it found 21% noncompliant36.

How it is sold

We could not confirm any seller offering orforglipron. On 7 October 2026, one seller’s website code listed ‘orforglipron’ and ‘o-glipron’ among product names hidden from logged-out visitors, alongside names such as retatrutide and semaglutide12. The same day, its /product/orforglipron/ address returned “Not Found”13. Finnrick listed no orforglipron vendors or tests11. Import Alert 66-80 lets FDA detain imported GLP-1 bulk ingredients from makers that are not on its Green List36. Legal prices: Lilly said self-pay Foundayo starts at $149 a month for the lowest dose, that people with commercial insurance may pay as little as $25 a month with its savings card, and that eligible Medicare Part D patients may pay $50 a month from 1 July 20263842. Under Lilly’s 6 November 2025 agreement with the US government, orforglipron “will be available with the lowest dose starting at $149 with additional doses up to $399”43.

Side effects and risks

  • Gut side effects are the most common: nausea, constipation, diarrhea and vomiting, along with indigestion, belly pain, bloating, burping, reflux and gas; headache, fatigue and hair loss were also reported in at least 5% of people3. In the two weight trials, 8% on Foundayo stopped because of side effects (6%, 9% and 10% on 5.5, 9 and 17.2 mg) versus 3% on placebo, mostly because of gut effects3. Severe gut reactions occurred in about 3% versus 1% on placebo, and the label does not recommend it for people with severe gastroparesis3.
  • Boxed warning: GLP-1 drugs that are active in rats and mice cause thyroid C-cell tumors in them. Orforglipron is not active in rodents and did not cause tumors in them, but it is active at the human receptor, and whether these tumors matter for people has not been determined. People with a personal or family history of medullary thyroid cancer or MEN 2 (multiple endocrine neoplasia type 2) must not use it3.
  • Other label warnings: inflamed pancreas (pancreatitis), kidney injury from dehydration after vomiting or diarrhea, low blood sugar when used with insulin or drugs that make the body release insulin, serious allergic reactions such as anaphylaxis and angioedema, worsening diabetic eye disease (retinopathy) in people with type 2 diabetes, gallbladder disease, and inhaling stomach contents during general anesthesia or deep sedation3.
  • Compared with oral semaglutide in ACHIEVE-3, orforglipron caused more gut side effects and more stopping because of side effects, and it raised pulse more (by 3.7 to 4.7 vs 1.0 to 1.5 beats per minute)5.
  • Interactions: with a strong CYP3A4 inhibitor the maximum dose is 9 mg, strong CYP3A4 inducers should be avoided, and simvastatin should not exceed 20 mg a day. Because it slows stomach emptying, it can change how other pills are absorbed, so the label advises people on birth control pills to switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase. The label does not recommend using it with another GLP-1 drug3.
  • Liver: exposure rose 1.7-fold in moderate and 4.6-fold in severe liver impairment, and the label does not recommend it in severe impairment (Child-Pugh C)3. FDA asked Lilly to report serious unexpected liver injury cases within 15 days for 5 years1. A pooled analysis of seven Phase 3 trials found no cases that met criteria for drug-induced liver injury28.
  • Pregnancy: it may cause fetal harm, and the label says to stop it when pregnancy is recognized3.
  • We found no public lab test of products sold outside prescription channels, so their content and purity are unknown11.
  • Anti-doping: it is not named on the Prohibited List, and our reading that an approved drug falls outside class S0 is not a WADA ruling910.

Documented harms

  • 30 Jul 2026FDA’s adverse event database (FAERS) held 728 reports naming orforglipron or Foundayo as of its 30 July 2026 update; 727 were received from April to June 2026. Twenty were classed as serious, and none reported a death. The most frequent terms were nausea (180), drug ineffective (122), constipation (81) and vomiting (75), and 2 reports listed pancreatitis. FAERS reports cannot show that a drug caused an event44.

Questions people ask

Is orforglipron a peptide?

No. It is a small nonpeptide molecule, unlike semaglutide and the other peptide drugs in its class, which are chains of amino acids214. It is covered here because it acts on the same GLP-1 receptor as semaglutide35.

Is orforglipron FDA approved?

Yes. FDA approved it as Foundayo on 1 April 2026 for weight management in adults with obesity, or with overweight and at least one weight-related condition13. As of 6 October 2026 the US label did not cover type 2 diabetes, for which Lilly has filed a separate submission3033.

Foundayo vs Wegovy pill: what’s the difference?

Foundayo was designed to be taken without food or water restrictions, and its label says to take it with or without food35. Wegovy tablets (semaglutide) must be taken on an empty stomach in the morning with up to 4 ounces of water, then nothing else for at least 30 minutes16. In separate trials, the top Foundayo dose cut weight by 11.2% at 72 weeks and the 25 mg semaglutide pill by 13.6% at 64 weeks; the trials differ, so the numbers cannot be compared directly223.

How much weight do you lose on orforglipron?

In ATTAIN-1, adults without diabetes lost 7.5%, 8.4% and 11.2% of their weight on average over 72 weeks on the three doses, versus 2.1% on placebo2. Adults with type 2 diabetes lost 5.1% to 9.6%, versus 2.5%6. The label reports these results under the Foundayo doses of 5.5, 9 and 17.2 mg3.

Do you have to take Foundayo on an empty stomach?

No. The label says to take it once daily with or without food and to swallow the tablet whole3. Food had no clinically relevant effect on how much of the drug was absorbed3.

Is orforglipron approved for type 2 diabetes?

Not in the US as of 6 October 2026: the Foundayo label covers weight management only, and Lilly’s 5 August 2026 results release listed a US submission for type 2 diabetes33033. The UK authorized it for both uses on 10 August 20264.

Is orforglipron banned in sports?

It is not named on WADA’s 2026 or 2027 Prohibited List, and as an FDA-approved drug it falls outside class S0 by our reading, so it is likely allowed1910. On 21 September 2026 WADA said the status of GLP-1 drugs will remain unchanged for 202741.

What is still unknown

  • Whether and when FDA will approve orforglipron for type 2 diabetes; Lilly listed a US submission in its 5 August 2026 results release.
  • The EU decision: EMA was still evaluating orforglipron in September 2026, and it was not among the CHMP meeting highlights of 14 to 17 September 2026.
  • Its status in Australia (TGA), Canada and Japan was not verified; the Japan and Canada submissions rest on one review article.
  • Whether it lowers heart attack and stroke risk: ACHIEVE-4 tested safety against insulin, and Lilly describes ATTAIN-OUTCOMES, a heart-outcomes trial in obesity, as ongoing.
  • Final ATTAIN-1 results: its registry record lists completion in October 2027.
  • Effects beyond about 2 years, and safety in pregnancy, breastfeeding and children.
  • What the 4 August 2026 labeling supplement changed.
  • Whether any gray-market seller actually offers orforglipron, and what such products contain.
  • No FDA warning letter, Health Canada notice or TGA notice naming orforglipron was found as of 7 October 2026; the searches were not exhaustive.
