PEG-MGF is sold online as a longer-lasting version of MGF, a lab-made peptide copied from part of a growth factor that muscles switch on when stretched123. We found no published study of PEG-MGF itself in people or animals45.

Quick answers
- What is it?
- A lab-made peptide sold as a long-acting form of MGF1. MGF copies the 24-amino-acid tail of a form of insulin-like growth factor 1 that muscles switch on when stretched23. Sellers say a chain called PEG is attached to make it last longer67.
- Does it work?
- Unknown. We found no published study of PEG-MGF in people or animals45. Animal studies of MGF without PEG reported some repair benefits89. A 2014 lab study by drug-company scientists found no apparent effect of MGF on muscle cells10.
- Is it safe?
- Unknown. FDA found no data on people exposed to it, and warns it may provoke immune reactions11. A 2026 medical review lists a theoretical concern that it could drive cell growth, based on lab research on MGF’s natural form in cancer1.
What it is
PEG-MGF is sold as MGF with a chain of polyethylene glycol (PEG) attached, a process called pegylation16. MGF, short for mechano growth factor, is named after one of several forms of insulin-like growth factor 1 (IGF-1). The IGF-1 gene can be spliced (its pieces cut and rejoined) in different ways, giving forms that share the same mature hormone but end in different tails, called E domains2223. The form behind MGF is called IGF-1Ec in people and IGF-1Eb in rats and rabbits, and rabbit studies found that muscle makes more of it when stretched32324. What is sold as MGF is not that whole protein but a lab-made copy of its tail: a 24-amino-acid peptide matching the E domain of the human form2123. A 2010 review noted that no such free peptide had been found in cells, animal tissues or body fluids2. Goldspink’s group reported in 2002 that the E-domain peptide made muscle precursor cells multiply, acting through a receptor other than IGF-1’s25. Drug-company scientists who tried to repeat such cell results in 2014 found no apparent effect10. Sellers say the PEG chain makes the peptide last longer in the body67, but we found no study that measured this, and a 2026 review lists no half-life for it1.
Geoffrey Goldspink and colleagues reported in 1996 that stretched rabbit muscle makes more of an unusual form of IGF-12124. In 1999 his group at the Royal Free and University College Medical School in London proposed calling it mechano growth factor, or MGF3. The name later passed to the synthetic 24-amino-acid E-domain peptide221. Anti-doping scientists described black-market MGF products in 201220. We found no source saying who first attached PEG to MGF, or when. Wells Pharmacy Network nominated PEG-MGF to FDA for use in compounding: the docket lists its nomination, with attachments titled ‘PEG-MGF Nomination Packet’, as received on 23 March 202126.
Benefits: what the research shows
We found no published study of PEG-MGF in people or animals. FDA says it ‘has not identified any human exposure data on drug products containing PEG-MGF administered via any route of administration’11. A ClinicalTrials.gov search on 8 October 2026 for PEG-MGF, mechano growth factor or IGF-1Ec returned 501 records; none gave MGF or PEG-MGF to anyone, and two only measure the body’s own MGF30. PubMed searches found no study that tested PEG-MGF itself and no trial that gave people any MGF peptide4531. A 2026 medical review lists ‘No peer-reviewed human studies’ for PEG-MGF and warns that effects seen with natural IGF-1Ec or E-domain peptides ‘cannot be assumed to apply to PEG-conjugated constructs’1. We therefore do not count studies of MGF without PEG as evidence for PEG-MGF. One mouse study delivered MGF from tiny rods of a PEG-based gel, but it did not attach PEG to the peptide28. A 2013 mouse study used MGF differently, as a tag attached through PEG chains to nanoparticles of chitosan, a sugar-based material, to help carry a test RNA from the nose to the brain. It tracked where the RNA went and looked for tissue damage, not for any effect of MGF itself32. A 2017 first-in-human study of RO5046013, a pegylated form of the whole IGF-1 hormone, in 62 healthy volunteers tested a different molecule and does not count either33. All other evidence concerns MGF peptides without PEG, in cells and animals, and it conflicts. Animal studies reported that MGF peptides helped transplanted human muscle precursor cells take hold in mice34, improved healing of a bone gap in rabbits at the higher of two doses8, improved healing of injured rat tendons35, protected brain cells in gerbils after a brief loss of blood flow to the brain36, and preserved heart function after heart attacks in sheep and mice9222829. In mice with bruised calf muscles and some immune cells (macrophages) removed, MGF injections reduced scarring but did not improve the regrowth of muscle fibers37. In a 2014 study by drug-company scientists, MGF at up to 500 ng/ml did not make mouse or human muscle cells multiply, while IGF-1 did; in heart muscle cells, neither the natural peptide nor a stabilized version showed any activating effect10. An editorial on that study summed up the authors’ view that MGF ‘on its own, even when modified to enhance its half-life’, was inert, while noting that ‘the last word about MGF has not been written yet’38.
Key human studies
No completed human studies were found.
Doses studied
Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.
In human studies
None. We found no study that gave PEG-MGF, or any MGF peptide, to people3031, and FDA found no human exposure data for PEG-MGF by any route11. A 2026 medical review found no peer-reviewed clinical study reporting a human dosing regimen1.
In animal studies
None for PEG-MGF: we found no animal study of it45. Studies of MGF peptides without PEG, as reported in the papers: mice with bruised calf muscles and macrophages (a type of immune cell) removed got 5 µg in 20 µl injected into each injured muscle right after the injury and 1 and 2 days later37. Mice after a heart attack got a stabilized MGF peptide at 4.5 mg/kg a day for 2 weeks from a pump implanted under the skin929. Rabbits with a 5-mm gap in the radius, a front-leg bone, got 28.5 or 57 µg/kg of body weight by injection for five days after surgery, and the abstract reports better healing with the higher dose8. These are animal doses of MGF peptides without PEG, a different molecule. They are not human doses, and we do not convert them.
What users report Anecdotal · not validated · not a recommendation
A dosing-protocol website (updated 20 September 2026) lists 200 to 500 µg injected under the skin once a day, and says ‘No clinical trials of PEG MGF have been completed’50. Bodybuilding-forum reports compiled by a 2026 medical review give 200 to 400 µg under the skin or into muscle, 2 or 3 times a week or daily1.
- Half-life
- No measurement found in people or animals1A 2026 medical review reports no half-life for PEG-MGF (‘t½ not reported’)1. Sellers’ claims conflict: one says PEG extends the half-life ‘from minutes to hours’, another says PEG gives it ‘a half-life of several days’67. We found no study that measured either. Anti-doping scientists found that an MGF variant without PEG, R23H, ‘has good stability’ in blood plasma in a test tube, without giving a half-life19.
- Routes
- Injection under the skin (the route community sites list; we found no human or animal data for PEG-MGF)
- Injection into muscle (in forum reports compiled by a 2026 review; we found no human or animal data for PEG-MGF)
- Injection into muscle or the heart, or infusion from a pump implanted under the skin (MGF peptides without PEG, in animals only)
Legal status
- United States
- PEG-MGF is not approved by FDA for any use. A search of FDA’s approved-drug database (Drugs@FDA, through openFDA) for the ingredient names ‘mechano growth factor’ and ‘PEG-MGF’ returned no match on 8 October 202612. FDA says compounded drugs containing PEG-MGF ‘may pose significant risk for immunogenicity for certain routes of administration’, meaning they may provoke immune reactions, ‘and may have complexities with regard to peptide-related impurities and API characterization’, that is, in confirming what the active ingredient is and what else it contains. It adds that it ‘has not identified any human exposure data on drug products containing PEG-MGF administered via any route of administration’ and ‘lacks important information regarding any safety issues raised by PEG-MGF, including whether it would cause harm if administered to humans’ (page current 22 April 2026)11. A search of FDA’s adverse event database (FAERS, through openFDA) for ‘PEG-MGF’, ‘mechano growth factor’ and ‘MGF’ returned no reports on 8 October 202641. Reporting is voluntary, so that does not show that no harm has occurred. FDA’s substance registry lists the peptide without PEG as MECHANO GROWTH FACTOR, with the identifier UNII Q86M4KXC2P27; that is a naming record, not an approval. In October 2020 a Kentucky compounding pharmacy, Tailor Made Compounding, pleaded guilty to distributing unapproved new drugs from 25 October 2018 to 1 April 2020; according to the plea agreement, the unapproved drugs it distributed included PEG-MGF, along with BPC 157, LL-37, MK 677 and others4256. In February 2021 the pharmacy was sentenced to three years of probation and forfeited $1,788,906.82, its 2019 sales of these products42.
- US pharmacies
- Under section 503A, pharmacies may compound only with bulk ingredients that have a USP or NF monograph, are part of an FDA-approved drug or are on FDA’s 503A bulks list46. FDA sorts the substances nominated for that list into three categories: under evaluation (Category 1), raising significant safety concerns (Category 2) and nominated without adequate support (Category 3)13. PEG-MGF is in none of them as of FDA’s update of 14 May 202613. Wells Pharmacy Network’s nomination of PEG-MGF was received on 23 March 202126, and FDA’s update of 29 September 2023 added ‘Mechano Growth Factor, Pegylated (PEG-MGF)’ to Category 243. On 14 April 2026 Wells withdrew its nominations of 12 peptides, PEG-MGF among them44. FDA now lists PEG-MGF under ‘Bulk drug substances nominated but withdrawn’, substances ‘previously in category 2’ (page current 22 April 2026)11. HHS Secretary Robert F. Kennedy Jr.’s post of 15 April 2026 on the 12 peptides leaving Category 2 listed ibutamoren mesylate rather than PEG-MGF45, but the withdrawal letter and FDA’s page both name PEG-MGF1144. A law firm that reported the April 2026 change stressed that removal from Category 2 ‘does not render these bulk drug substances eligible for compounding under section 503A’, and that they stay outside FDA’s interim enforcement discretion policy, ‘which currently extends only to substances in Category 1’14. MGF without PEG, listed as ‘Mechano growth factor (MGF)’, has been in Category 3 since the 29 September 2023 update and is still there on the list updated 14 May 20261343. FDA says Category 3 substances ‘are not eligible for the policy that applies to substances in category 1’ and that it ‘would consider taking action against a compounder’ using them46. PEG-MGF was not on the agenda of the Pharmacy Compounding Advisory Committee meeting of 23 July 2026 and 24 July 20261554. FDA says the committee will discuss PEG-MGF, with GHK-Cu, LL-37, dihexa acetate and melanotan II, at a meeting before the end of February 2027, listing them as substances ‘being considered for inclusion on the 503A bulks list’53. Two law firms reported the same plan on 16 April 2026 and 27 July 20261415. The committee’s advice is not binding, and FDA must still go through formal rulemaking before adding any substance to the list14. On 8 October 2026 FDA’s page for the meeting still said its time and place ‘will be scheduled in the coming months’, and a Federal Register search that day found no notice of the meeting, which FDA says it intends to publish4753. Neither PEG-MGF nor MGF is on FDA’s 503B list for outsourcing facilities, updated 21 March 202548.
- Other countries
- Australia: the Poisons Standard in force from 1 October 2026 does not name PEG-MGF or MGF. It lists ‘INSULIN-LIKE GROWTH FACTORS’ as prescription-only (Schedule 4) and in Appendix D clause 5, which covers poisons that must not be possessed without authority, such as a prescription40. Whether that group entry covers MGF or PEG-MGF was not verified. Anti-doping scientists wrote in 2012 that MGF ‘has not yet been approved for therapeutic use’20. We found no approval of PEG-MGF as a medicine: FDA’s approval database has no match12, and FDA says it has not identified any human exposure data on drug products containing PEG-MGF11. Its status in the UK, the EU and Canada was not verified.
- Sport
- Prohibited at all times, in and out of competition. Class S2.3 of the 2026 Prohibited List names ‘Mechano growth factors (MGFs)’, next to ‘Insulin-like growth factor 1 (IGF-1, mecasermin) and its analogues’, and class S2 also covers ‘other substances with similar chemical structure or similar biological effect(s)’16. The List does not name PEG-MGF separately; because PEG-MGF is sold as MGF with PEG attached, we read the ‘Mechano growth factors (MGFs)’ entry as covering it1616. All substances in class S2 are non-Specified, meaning they are not among those WADA considers more likely to be taken for reasons other than sport performance16. The 2027 List, in force from 1 January 2027, keeps the same entry39. Anti-doping scientists say MGF has been considered prohibited since 200521, and a urine test published in 2024, which its authors validated as an initial testing procedure in doping control, covers ‘human MGF’ and ‘MGF-Goldspink’49.
Quality and the gray market
What testing finds
We found no published independent purity test of PEG-MGF. As of 8 October 2026, Finnrick, an independent testing service that publishes laboratory results for gray-market peptides, listed no published PEG-MGF tests and no rated vendors17. Seller claims cannot be checked: one claims purity of at least 99%6, and another claims ‘98% + Purity’7. The only published analysis of a product sold as PEG-MGF that we found is a 2017 chemistry-methods paper: it used ‘PEG MGF’ bought from a research-peptide seller as test material and reported that ‘Non-PEGylated MGF cations can be observed’, that is, MGF without PEG attached; it did not test purity or report whether any PEG-bound peptide was present51. Anti-doping laboratories have found altered MGF peptides, without PEG, in black-market and seized products. In 2012 both of two black-market preparations were ‘C-terminal amidated analogues of human MGF’, with the end of the chain chemically modified20; a 2017 study found an ‘R23H’ variant in confiscated vials19; and in 2014 a ‘full-length MGF’ offered to athletes turned out to be a protein closely related to IGF-1Ec, with its last amino acid removed and an arginine replaced by histidine21.
How it is sold
PEG-MGF is sold online as powder in 2 mg or 5 mg vials labeled for research use only6752. On 8 October 2026 one seller listed a 2 mg vial at $40.00 and another at a ‘special price’ of $39.96 against a ‘regular price’ of $49.9567. The same day, one research-peptide shop’s popularity-sorted list ranked its 5 mg PEG-MGF 38th of 104 products, at $63.00, ahead of plain MGF (70th, at $50.00) and a product labeled ‘MGF IGF-1 EC’ (104th, at $145.00); the shop says its products ‘are not for human consumption’52. Sellers’ pages claim that PEG-MGF ‘directly assists with the up-regulation of protein synthesis and nitrogen retention’ and that it ‘has been investigated in preclinical models’67, although we found no published study of PEG-MGF itself45. One manufacturer’s page lists ‘INDICATIONS’ including tendon, ligament and muscle repair, muscle wasting, stroke and heart attack, and gives absorption figures ‘in healthy male and female adults’ without naming any study18; FDA says it has identified no human exposure data for PEG-MGF11.
Side effects and risks
- Unknown human safety. We found no study that gave PEG-MGF to people, and FDA says it lacks information on ‘whether it would cause harm if administered to humans’4511.
- Immune reactions and impurities. FDA warns that compounded PEG-MGF may provoke immune reactions with some routes of use and may carry peptide-related impurities11.
- Theoretical cancer concern. A 2026 medical review lists a ‘Theoretical proliferative concern’, a possible push toward cell growth, based on preclinical research on MGF’s natural form in cancer1. We found no study of PEG-MGF’s long-term safety in animals or people45.
- Unknown product content. Finnrick, an independent testing service, had published no tests of PEG-MGF as of 8 October 2026, and anti-doping laboratories have found altered MGF peptides in black-market and seized products17192021.
- Anti-doping. Mechano growth factors are banned at all times under WADA class S2.3, and anti-doping scientists have published a validated urine test that covers MGF1649.
- Legal. US pharmacies have no lawful route under section 503A to compound PEG-MGF, and a Kentucky compounding pharmacy that distributed it, among other unapproved drugs, was sentenced in 2021131442.
Questions people ask
Is PEG-MGF FDA approved?
No. No FDA-approved drug contains PEG-MGF, and US pharmacies have no lawful route to compound it121314. FDA plans to ask its compounding advisers, at a meeting before the end of February 2027, whether to add it to the list of substances pharmacies may use53.
What is PEG-MGF used for?
It is promoted for muscle recovery and growth: a dosing website gives its goal as supporting ‘muscle recovery and regeneration’, and one manufacturer’s page presents tendon, ligament and muscle repair, muscle wasting, stroke and heart attack as medical indications1850. We found no test of any of these uses with PEG-MGF in people or animals4511.
MGF vs PEG-MGF: what’s the difference?
MGF is a lab-made 24-amino-acid peptide copied from the tail of IGF-1Ec, and PEG-MGF is sold as the same peptide with a PEG chain attached121. Sellers say PEG makes it last longer, but a 2026 review found no reported half-life, and warns that results with MGF peptides ‘cannot be assumed to apply’ to PEG-bound versions16.
Does PEG-MGF build muscle?
Nobody knows: we found no study of PEG-MGF in people or animals45. MGF peptides without PEG made muscle precursor cells multiply in some lab studies, but drug-company scientists found no apparent effect on muscle cells in 2014102534. A 2026 medical review says claims that PEG-MGF ‘reliably accelerates recovery or improves body composition in humans remain unproven’1.
Is MGF the same as IGF-1?
No. The mature IGF-1 hormone is 70 amino acids long, while MGF copies a separate 24-amino-acid tail found at the end of unprocessed IGF-11038. Goldspink’s group reported that the tail acts through a different receptor, but in drug-company tests MGF had no apparent effect on muscle cells where IGF-1 did1025.
Is PEG-MGF banned in sports?
Yes. WADA’s Prohibited List bans ‘Mechano growth factors (MGFs)’ at all times, in and out of competition, and we read that entry as covering PEG-MGF, which is sold as MGF with PEG attached116. The 2027 List keeps the same entry, and anti-doping scientists have published a validated urine test that covers MGF3949.
What is still unknown
- Whether PEG-MGF does anything in people or animals. We found no published study of it.
- What gray-market PEG-MGF vials contain: which peptide sequence, how large a PEG chain, where it is attached and how much of the peptide carries PEG. Finnrick, an independent testing service, had published no tests of PEG-MGF as of 8 October 2026.
- Whether the body makes a free MGF peptide at all. A 2010 review found none identified in cells, tissues or body fluids.
- How long PEG-MGF lasts in the body. Sellers’ claims range from hours to several days, and we found no study that measured it.
- When FDA’s compounding advisers will meet on PEG-MGF, and what FDA will then decide. FDA has said only that the meeting will be before the end of February 2027, and had set no date by 8 October 2026.
- Why sellers and three mouse heart studies use an MGF sequence ending in EEHK, when the human sequence, in both FDA’s substance registry and UniProt, ends in EERK.
- Whether the Australian Poisons Standard’s group entry for insulin-like growth factors covers MGF or PEG-MGF.
- PEG-MGF’s legal status in the UK, the EU and Canada.
