GHRP-2 is a lab-made peptide that makes the body release growth hormone1. Japan approved it in 2004 as a one-dose test for growth hormone deficiency12. We found no trial of it for muscle gain or fat loss34.

Quick answers
- What is it?
- A lab-made chain of six amino acids that makes the body release growth hormone1. Japan approves it only as a one-dose test for growth hormone deficiency12. It is also sold online, often in blends, for muscle gain and fat loss45.
- Does it work?
- As a test, yes: it caught 85.3% of severe cases among 502 patients and ruled out 88.5% of the others2. For growth in short children, a nasal spray did no better than a dummy spray6. We found no trial of muscle gain or fat loss4.
- Is it safe?
- Before approval, about a third of 227 people in trials had side effects, such as a warm feeling; none was serious27. FDA cites serious reports, including deaths of critically ill study subjects, though a causal link is unproven8. Long-term safety is unknown4.
- Is it legal?
- Approved in Japan as a test, not in the US19. By our reading, US pharmacies may not make it101112. Prescription-only in Australia, and illegal to hold without one13. Canada has seized it as an unauthorized drug14. Banned in sport at all times1516.
- What is in the vials?
- Finnrick, which publishes lab tests of peptide samples, had no GHRP-2 results on 7 October 202617. US pharmacies recalled 30 sermorelin blends containing it from 2012 to 2019, all over sterility doubts18. Customs seizures in Denmark held look-alike copies with an extra amino acid19.
What it is
GHRP-2 (growth hormone-releasing peptide-2), also called pralmorelin, is a lab-made peptide of six amino acids. Three of them are mirror-image (D-) forms, including D-2-naphthylalanine, an unnatural building block not found in human proteins18. Like ghrelin, the ‘hunger hormone’, it switches on the ghrelin receptor (also called the growth hormone secretagogue receptor) and is thought to act mainly through the hypothalamus, so that the pituitary gland releases a burst of growth hormone (GH)17. After one dose into a vein, GH usually peaks within 15 to 60 minutes1. GHRP-2 also raises appetite and food intake2021, and it raises the stress hormones ACTH and cortisol and the hormone prolactin22. In Japan it is sold as GHRP KAKEN100 Injection, a one-dose test for growth hormone deficiency1.
GHRP-2 grew out of the work of C.Y. Bowers and colleagues at Tulane University in the US, who in 1977 found peptides that selectively released GH from the pituitary in laboratory experiments and in 1984 discovered GHRP-67. Kaken Pharmaceutical then began joint research with Bowers’s group and synthesized KP-102 (GHRP-2), which released GH more strongly7. It also carried the Wyeth-Ayerst code GPA-74823. After the receptor it acts on was described in 1996 and its natural partner, ghrelin, in 1999, GHRP-2 came to be seen as a ghrelin mimic7. Kaken developed it as a quicker test for severe GH deficiency: in its trials GH peaked about 30 minutes after a dose on average and within 60 minutes in everyone, vs about 60 to 120 minutes for older approved tests, so blood sampling could end at 60 minutes7. Japan approved it on 22 October 2004, and its international nonproprietary name is pralmorelin27.
Benefits: what the research shows
GHRP-2’s strongest evidence is as a diagnostic test. In 77 healthy adults and 58 patients whose insulin tolerance test, the standard test, showed severe GH deficiency, one 100 µg dose into a vein after an overnight fast produced a GH peak within 60 minutes in everyone. Peaks averaged 84.6 µg/L in healthy people and 1.36 µg/L in patients, and a cut-off of 15 µg/L matched the standard test’s 3 µg/L26. In Kaken’s post-marketing survey of 502 patients, 249 of them children, the test picked up 85.3% of the severe cases that doctors diagnosed with at least two other stimulation tests and correctly ruled out 88.5% of the rest2. In the confirmatory trial before approval, 23 of 50 people had side effects after GHRP-2, all mild, vs 32 of 50 after the insulin test, mainly symptoms of low blood sugar, 2 of them serious7. Treatment studies were small, and the one placebo-controlled growth trial was negative. In it, 126 short children with GH deficiency used a GHRP-2 nasal spray or a placebo spray twice daily for 48 weeks; height gain did not differ and IGF-1, the blood marker of GH action, did not change6. Earlier small studies without a placebo group reported faster growth on nasal or injected GHRP-2, also without a rise in IGF-12728. Infusions under the skin raised food intake: by 35.9% in 7 lean men, and by 10.2% on a low dose and 33.5% on a high dose in 19 lean and obese adults2021. In 10 older adults with low GH output, a continuous infusion under the skin for 30 days raised GH pulses and IGF-1 throughout29. In critically ill adults, a 21-hour infusion raised GH 4- to 6-fold and IGF-1 by 61% within 24 hours30. Over 5 days in critically ill men, GHRP-2 alone restored GH output and normalized IGF-1 but did not lower urea production, a marker of protein breakdown; combinations with the thyroid-stimulating hormone TRH did31. We found no trial of GHRP-2 for muscle gain, fat loss, recovery or anti-aging. The closest is a chart review of 105 men on testosterone who wanted more lean mass and less fat and were prescribed a blend of GHRP-2, GHRP-6 and sermorelin. In the 14 who met strict criteria, IGF-1 rose from 159.5 to 239.0 ng/mL, and the abstract reports no body-composition results3. A 2026 review concluded that beyond diagnosis, ‘broader therapeutic use of GHRP-2 remains limited’4. We rate GHRP-2 B because controlled human trials with public results exist, but none tested the uses it is sold for.

Doses studied
Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.
In human studies
Japan’s approved test dose is 2 µg/kg for ages 4 to 17 (100 µg above 50 kg) and 100 µg for adults, injected slowly into a vein while fasting1. Studies in adults used 100 µg IV once for diagnosis26; 0.1 or 1 µg/kg per hour as a 270-minute infusion under the skin for appetite2021; a 1 µg/kg bolus then 1 µg/kg per hour for 21 hours, or 1 µg/kg per hour for 5 days, in critically ill patients3031; and 1 µg/kg per hour under the skin for 30 days in older adults29. Studies in children used a nasal spray of 50 to 200 µg twice daily for 48 weeks6, nasal doses of 5 to 15 µg/kg two or three times a day27, 0.3 to 3 µg/kg per day under the skin28 and 900 µg/kg by mouth twice daily for 12 months23. An anti-doping study gave one 10 mg oral dose37. These studies measured hormones, appetite, growth or detection, not muscle or fat4.
In animal studies
Pigs: single IV doses of 2 to 100 µg/kg raised GH in proportion to dose, and 30 µg/kg under the skin once daily for 30 days raised daily weight gain by 22% and feed efficiency by 21%, with a partly weaker GH response by day 1039. Rats and dogs: IV doses released GH in proportion to dose1.
What users report Anecdotal · not validated · not a recommendation
A 2026 medical review compiled bodybuilding-forum reports: 100 to 150 µg under the skin, 2 or 3 times a day4.
- Half-life
- About 25 to 41 minutes (0.42 to 0.69 hours) after a dose into a vein in healthy men1Elimination half-life after single IV doses of 0.3, 1.0 or 3.0 µg/kg in 18 healthy men. No metabolites were found in plasma after a 100 µg IV dose; by 24 hours about 2% of the dose appeared unchanged in urine and about 4% as metabolites, and excretion is thought to be mainly through bile, as in rats and dogs1. In 10 children given 1 µg/kg IV, the half-life was 0.55 hours and GH peaked at about 25 minutes (0.42 hours)38.
- Routes
- Intravenous injection (the approved one-dose test in Japan, and hospital studies)
- Injection or infusion under the skin (research studies; the usual gray-market route)
- Nasal spray (tested in children for growth)
- By mouth (tested in children and in an anti-doping study)
Legal status
- United States
- Not approved for any use. Drugs@FDA searches for pralmorelin and for GHRP returned no matches932. Kaken’s approval dossier says GHRP-2 was studied in US clinical research under an FDA investigator IND (No. 38149) but was not marketed7, and in 2014 Japan’s regulator noted it was not approved or sold in any other country2. It is not a federally controlled substance33. FDA’s Import Alert 66-41 lists ‘GHRP2’ and ‘GHRP6’ from a Beijing firm, in entries dated 7 August 201234. In a 2024 briefing, FDA found research-use-only powders sold online that combined CJC-1295 with GHRP-2, alone or with ipamorelin35.
- US pharmacies
- Under section 503A, pharmacies may compound only with bulk ingredients that have a USP or NF monograph, are part of an FDA-approved drug or are on FDA’s 503A bulks list11. GHRP-2 is in no FDA-approved drug9, and FDA’s 503A list, updated 14 May 2026, places it in Category 3: substances ‘nominated with insufficient supporting information for FDA to evaluate them’1011. FDA says these are not eligible for the interim policy that covers Category 1 substances, and that it ‘would consider taking action against a compounder’ that uses one11. Our reading is that US pharmacies have no lawful route to compound it under 503A. For outsourcing facilities (503B), injectable and nasal GHRP-2 has been in Category 2, substances that raise significant safety risks, since 29 September 2023; other routes are in Category 1, ‘under evaluation’812. FDA cites possible immune reactions from clumping (aggregation) and impurities, an unnatural amino acid that makes the substance harder to characterize, and reports of serious adverse events, including increased insulin needs, deaths of critically ill study subjects, infection and pancreatitis, ‘though causality has not been established’8.
- Other countries
- Japan: approved on 22 October 2004 as GHRP KAKEN100 Injection (Kaken Pharmaceutical), a prescription test for diagnosing growth hormone deficiency12. Its 2014 re-examination found that none of the legal grounds for refusing approval applied2. Australia: the Poisons Standard in force from 1 October 2026 lists pralmorelin (GHRP-2), and GHRPs as a group, in Schedule 4 (prescription only) and in Appendix D, which makes possession without authority, such as a prescription, illegal13. Canada: Health Canada says injectable peptides are regulated as prescription drugs; in 2025 it listed GHRP-2 among unauthorized products seized from or sold by Canada Peptide14. UK and EU: not verified.
- Sport
- Prohibited at all times, in and out of competition: S2.2.4 of the 2026 List names ‘GHRP-2 (pralmorelin)’ among GH-releasing peptides, and it is not a Specified substance15. The 2027 List, in force from 1 January 2027, keeps the same wording16. Labs can detect it: after one nasal dose, GHRP-2 and two breakdown products were found in urine for up to 47 hours36, and after a 10 mg oral dose a known metabolite was detectable for more than 20 hours37. Products seized by Danish customs held a glycine-extended version, and the authors urged labs to update their tests19.
Quality and the gray market
What testing finds
Finnrick, a service that publishes lab results for peptide samples, listed no GHRP-2 tests and no rated vendors on 7 October 202617, so there are no independent data on what gray-market vials contain. FDA’s recall records name GHRP-2 in 30 compounded sermorelin products recalled by 9 US pharmacies between 21 May 2012 and 12 September 2019, all for lack of sterility assurance18. Doping-control chemists found glycine-extended GHRP-2, a look-alike with an extra amino acid, in products seized by Danish customs19. FDA notes that GHRP-2’s unnatural amino acid makes the substance harder to characterize8.
How it is sold
Sold online as ‘research use only’ powder, alone and in blends. On 7 October 2026 one US vendor listed GHRP-2 in 5 mg and 10 mg vials and in 5 blends with CJC-1295 or Mod GRF 1-29, ipamorelin, sermorelin or GHRP-6, under the statement ‘All products are for laboratory research use only’5. FDA’s 2024 CJC-1295 briefing also found research-use-only powders combining CJC-1295 with GHRP-235. Some doctors have prescribed it: a chart review describes 105 men on testosterone, seeking more lean mass and less fat, who were prescribed GHRP-2 with GHRP-6 and sermorelin three times daily3. FDA recall records show US compounding pharmacies making sermorelin blends with GHRP-2 from at least 2012 to 201918. In 2025 Health Canada warned about unauthorized injectable GHRP-2 seized from or sold by Canada Peptide14.
Side effects and risks
- Unknown long-term safety: the approved use is a single test dose, and a 2026 review found no long-term safety data14.
- Hunger: infusions raised food intake by about a third in healthy adults2021, and 7 of 10 children taking it by mouth reported a significant increase in appetite in the first 6 months23.
- Stress hormones: single IV doses raised ACTH and cortisol about as much as a standard CRH test, and raised prolactin22.
- Serious reports cited by FDA: increased insulin needs, deaths of critically ill study subjects, infection and pancreatitis; FDA says causality has not been established8.
- Immune reactions and impurities: FDA warns that injected or nasal GHRP-2 could provoke immune responses because of clumping and peptide impurities8.
- Side effects listed on the Japanese label for the test dose: in 5% or more, a feeling of warmth (16.0%), stomach rumbling and a rise in white blood cells; in 0.1% to 5%, low blood pressure, nausea, sleepiness, sweating, thirst, hunger and tiredness; at unknown frequency, dizziness, headache, flushing and a raised liver enzyme (ALT)1.
- Pregnancy: the Japanese label says not to give it to women who are or may be pregnant, and it passes into milk in rats1.
- Pituitary tumors: the label warns that similar drugs given to people with pituitary adenomas have been reported to cause pituitary apoplexy (sudden bleeding or loss of blood supply in the gland), with vision problems, headache and vomiting1.
- Contaminated or wrong products: compounded blends containing GHRP-2 were recalled for lack of sterility assurance, and seized products have held look-alike versions1819.
- Legal and anti-doping risk: banned in sport at all times and detectable in urine for up to 47 hours after a nasal dose; in Australia, possession without a prescription is illegal131536.
Documented harms
- 5 Feb 2014Japan’s regulator reported 2 patients with 5 serious side effects after the GHRP-2 test during the 8-year re-examination period, all of whom recovered: impaired consciousness, facial pallor, abdominal pain, sweating and raised blood pressure, one of each. The 3 events the label did not predict (impaired consciousness, pallor and raised blood pressure) occurred in one patient and resolved the same day. That patient had a pituitary tumor, adrenal insufficiency, ventricular fibrillation and high blood pressure and took several drugs; the reporting doctor suspected missed morning doses, including hydrocortisone, and Kaken linked the blood-pressure rise more to a skipped blood-pressure drug. Kaken said a link to GHRP-2 could not be ruled out, but other causes were possible2.
- 10 Oct 2016Five US consumer reports to FDA’s adverse event system (FAERS), received from 10 October 2016 to 1 December 2016, involved compounded sermorelin products that also contained GHRP-2 and GHRP-6. A 64-year-old woman was hospitalized with acute pancreatitis, nausea and vomiting, and reported a problem with the product’s physical consistency. A 39-year-old man using a ‘sermorelin acetate/GHRP injectable kit’ reported pain, infection and a repeated surgical procedure and was hospitalized; both reports were marked life-threatening. Three women aged 43 to 59 reported hives, itching, a pustular rash, burning, pain or fever, and one flagged a product quality problem. FAERS reports are unverified and cannot show which product, if any, caused an event40.
- 22 May 2019A Japanese physician’s report to FAERS, based on a 2019 conference abstract, describes a 28-year-old man with an acute liver disorder (coded as worsening fatty liver) after surgery for a non-functioning pituitary tumor; GHRP-2 and hydrocortisone were listed as suspect drugs. A report like this does not show that GHRP-2 caused the problem40.
- 29 Sep 2023FDA placed injectable and nasal GHRP-2 in the 503B category of compounding substances that raise significant safety risks, citing reports of serious adverse events including increased insulin needs, deaths of critically ill study subjects, infection and pancreatitis, ‘though causality has not been established’8.
Questions people ask
Is GHRP-2 FDA approved?
No. No FDA-approved drug contains it, and by our reading FDA’s compounding rules do not let US pharmacies make it91011. Japan approved it in 2004, but only as a one-dose test for growth hormone deficiency12.
What is pralmorelin used for?
Pralmorelin is the official generic name of GHRP-2. In Japan, doctors inject one dose into a vein while the patient is fasting and measure growth hormone over the next hour to diagnose severe growth hormone deficiency1.
Does GHRP-2 increase appetite?
Yes, in studies. Seven lean men ate 35.9% more at a buffet after a 270-minute infusion than after saline, and lean and obese adults ate 10.2% more on a low dose and 33.5% more on a high dose2021.
Does GHRP-2 raise cortisol?
Yes. In healthy young adults, single IV doses raised the stress hormones ACTH and cortisol about as much as the standard CRH test22. Researchers have studied that effect as a test for adrenal failure caused by pituitary or hypothalamic disease41.
Does GHRP-2 make kids grow taller?
Not in the only placebo-controlled trial we found: 126 short children with growth hormone deficiency used a GHRP-2 nasal spray or a dummy spray for 48 weeks, and growth did not differ6. Smaller studies without a placebo group reported faster growth2728.
What is still unknown
- Kaken’s 2004 dossier reports side effects in 74 of 227 people (32.6%) in pre-approval trials, plus lab-test changes in 13, while the 2014 re-examination report gives 86 of 227 (37.9%); the second figure may combine the two, but neither document says so.
- We found no study that tested GHRP-2 under the skin in adults for muscle, fat, recovery or aging outcomes.
- FDA has not said which reports of deaths, infection or pancreatitis it relied on, so their link to GHRP-2 cannot be judged.
- Uncontrolled studies reported faster growth in children, but the one placebo-controlled trial found none; whether dose, route or patient selection explains the gap is unknown.
- No independent purity or content testing of gray-market GHRP-2 has been published.
- UK and EU status, and whether a USP or NF monograph exists for GHRP-2, were not verified.
