Quick answers

What is it?
A lab-made chain of six amino acids, built from GHRP-6, that makes the body release growth hormone16. It is sold online as a research chemical, alone or blended with CJC-129578.
Does it work?
It strongly raises growth hormone, but the response weakens with daily use19. In 12 older adults given 16 weeks of injections, body fat and lean mass did not change significantly10. Its heart studies were small, one-dose tests2.
Is it safe?
Short studies reported few side effects1112, but it raises the stress hormones ACTH and cortisol and the hormone prolactin1314. It also reduced deep sleep15. Long-term safety is unknown5.
Is it legal?
Not approved in the US, and by our reading US pharmacies may not make it41617. Prescription-only in Australia, and illegal to hold without one18. Banned in sport at all times1920.
What is in the vials?
Finnrick, which publishes lab tests of peptide samples, had no hexarelin results on 7 October 202621. Health Canada warns that unauthorized injectable peptides may hold contaminants or ingredients not on the label22.

What it is

Hexarelin is a lab-made peptide of six amino acids, His-D-2-methyl-Trp-Ala-Trp-D-Phe-Lys-NH223. It was made from GHRP-6 by replacing a tryptophan with the more chemically stable 2-methyl-tryptophan624. Like other growth hormone-releasing peptides, it switches on the ghrelin receptor (also called the growth hormone secretagogue receptor), so the pituitary gland releases bursts of growth hormone (GH)5. It also raises prolactin, ACTH and cortisol1314. In rat heart tissue it binds a second receptor, CD3625, and in small human studies one dose briefly made the heart pump harder2. It is not approved as a medicine in the US426. The anti-doping List and PubChem call it examorelin1927.

In 1994 Deghenghi and colleagues reported hexarelin as a new growth hormone-releasing peptide: GHRP-6 with a tryptophan replaced by the chemically more stable 2-methyl-tryptophan624. It strongly raised GH in 10-day-old rats. Given into a vein in adult rats it worked about as well as GHRP-6; given under the skin it caused a long-lasting release and was slightly more effective. The authors suggested it could become a diagnostic or treatment tool6. The same year, a study in healthy volunteers found that it raised GH when given into a vein, under the skin, into the nose and by mouth1. Through the 1990s and early 2000s it was studied in children and adults with growth hormone deficiency28, in short children given it into the nose to speed growth11 and for its effects on the heart23. A 1997 review by Deghenghi noted that absorption by mouth rarely exceeded 1% of the dose, and that newer drugs of this class were non-peptides with reasonable absorption by mouth24.

Benefits: what the research shows

Hexarelin strongly raises GH in people. In 12 healthy young volunteers, an IV dose of 1 µg/kg released about twice as much GH as the body’s own GH-releasing hormone (GHRH); compared with an IV dose, it kept about 77% of its activity under the skin, 4.8% into the nose and 0.3% by mouth1. In a double-blind, placebo-controlled study in 12 healthy men, GH peaked about 30 minutes after an IV dose, and 2 µg/kg gave close to the maximum response23. It is not specific to GH: it also raises prolactin, ACTH and cortisol1314, and in 7 young volunteers four nighttime doses of 50 µg reduced deep sleep compared with placebo15. With daily use the GH response weakens. In 12 healthy older adults injected twice daily for 16 weeks, the GH response to a test dose fell by about 45% and recovered 4 weeks after stopping; IGF-1, the blood marker of GH action, did not change significantly, and neither did body fat, lean mass or bone density. That study had no comparison group910. In 8 short children, doses into the nose three times daily for up to 8 months raised IGF-1 and growth speed (from 5.3 to 8.3 cm a year) and reduced skinfold thickness11. In 7 short children treated the same way, the GH response to a test dose roughly halved within 7 days, but growth still sped up29. The heart studies were small, gave one IV dose and measured the left ventricular ejection fraction (LVEF), the share of blood the heart’s main pumping chamber pushes out with each beat. In 7 male volunteers, hexarelin raised LVEF from 64.0% to 70.7% for up to an hour, while GH itself, at a dose that raised GH levels as much, had no effect30. A 2 µg/kg dose raised LVEF in healthy adults and in GH-deficient patients but not in 12 patients with dilated cardiomyopathy2; another study found a rise in 5 patients whose heart damage came from blocked arteries (22.6% to 26.2%) but not in 8 with dilated cardiomyopathy31. In 24 men having bypass surgery under anesthesia, hexarelin, unlike GHRH, GH or placebo, raised LVEF and cardiac output within 10 minutes, for up to 90 minutes3. Some participants may appear in more than one report: one study compared its patients with healthy and GH-deficient groups ‘previously studied with the same methodology’31. None of these studies tested repeated doses or measured symptoms or survival. In rats, hexarelin protected isolated hearts when blood flow was stopped and restored, and improved pumping after a heart attack3233. We found no controlled trial of hexarelin for muscle gain, fat loss, recovery or anti-aging; a 2026 review calls the human evidence for body-composition effects limited5. We rate it B because controlled human studies with public results exist, but none tested the uses it is sold for.

Rahim et al., J Clin Endocrinol Metab 199810PubMed · DOI 16-week study in healthy older adults with no comparison group; the companion report with the same GH results gives 12 participants 12 1.5 µg/kg under the skin twice daily for 16 weeks The GH response to a test dose fell from 19.1 to 10.5 µg/L·h by week 16 and was back to 19.4 four weeks after stopping. IGF-1 did not change significantly, and total body fat, lean body mass and bone density had not changed significantly at week 16910. The cortisol response fell during treatment and recovered after it; prolactin did not change significantly35.
Laron et al., Clin Endocrinol (Oxf) 199511PubMed · DOI Study in prepubertal short children aged 4 to 11.6, with no comparison group 8 Into the nose, 60 µg/kg three times daily for up to 8 months IGF-1 rose from 10.4 to 14.1 nmol/L and growth speed from 5.3 to 8.3 cm a year; skinfold thickness fell despite weight gain. The authors described the effects as ‘seemingly without undesirable side-effects’11.
Imbimbo et al., Eur J Clin Pharmacol 199423PubMed · DOI Double-blind, placebo-controlled, rising-dose study in healthy adult men 12 Single IV doses of 0.5, 1 and 2 µg/kg, and placebo GH rose with dose, peaking about 30 minutes after injection (mean peaks 26.9, 52.3 and 55.0 ng/mL vs 3.9 on placebo) and returning to baseline within 240 minutes; the 2 µg/kg dose was very close to the maximum response23.

GH rose in healthy people and in cardiomyopathy but not in GH deficiency, so the authors judged the heart effect independent of GH2.
Broglio et al., Eur J Pharmacol 20023PubMed · DOI Controlled study in men with coronary artery disease during bypass surgery under general anesthesia, comparing hexarelin with GHRH, GH or placebo (group sizes not given in the abstract) 24 One IV dose of hexarelin 2 µg/kg (GHRH 2 µg/kg, GH 10 µg/kg) Hexarelin raised LVEF, cardiac index and cardiac output from 10 minutes, lasting up to 90 minutes, and lowered wedge pressure, while mean arterial pressure rose. GHRH, GH and placebo had no effect on the heart, and hexarelin raised GH only slightly3.

Doses studied

Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.

In human studies

Single IV doses of 0.5 to 2 µg/kg in healthy adults, with 2 µg/kg giving close to the maximum GH response23, and 2 µg/kg IV in the heart studies23. Under the skin: single doses of 1.5 and 3 µg/kg1, 1.5 µg/kg two or three times in one day40, and 1.5 µg/kg twice daily for 16 weeks in older adults10. Into the nose: 20 µg/kg once141, 1.25 mg (about 18 µg/kg) three times daily for 8 days in older adults12, and 60 µg/kg three times daily for up to 8 months in short children11. By mouth: single doses of 20 and 40 mg1, and 20 mg three times daily for 15 days in older women12. Apart from skinfold changes in children11 and the unchanged body composition after 16 weeks10, these studies measured hormones, growth, sleep or heart function, not muscle or fat.

In animal studies

Rats: 80 µg/kg under the skin, given for 7 days, protected isolated hearts when blood flow was stopped and restored32; 10 or 100 µg/kg a day under the skin, split into two doses, for 2 weeks starting 4 weeks after a heart attack, with the higher dose raising cardiac output33. Mice: one dose by mouth 30 minutes after a heart attack preserved pumping in the chronic phase; the abstract does not give the dose42. Dogs: 1 µg/kg into a vein and 1 to 100 µg/kg under the skin to measure drug levels39.

What users report Anecdotal · not validated · not a recommendation

A 2026 medical review compiled bodybuilding-forum reports: 100 to 200 µg under the skin, once or twice a day5.

Half-life
No human value found; about 120 minutes in dogs after a dose into a vein39We found no published measurement of how long hexarelin itself stays in human blood. In dogs given 1 µg/kg into a vein and measured with an antibody-based assay, the terminal half-life was 120 minutes39. In healthy men given an IV dose, GH peaked at about 30 minutes and fell back to baseline within 240 minutes with a half-life of about 55 minutes; that figure describes growth hormone, not hexarelin23.
Routes
  • Intravenous injection (hormone tests and heart studies)
  • Injection under the skin (studied in adults; the usual gray-market route)
  • Into the nose (studied in children and older adults)
  • By mouth (studied, but only about 0.3% as active as an IV dose)

Quality and the gray market

What testing finds

Finnrick, a service that publishes lab results for peptide samples, listed no hexarelin tests and no rated vendors on 7 October 202621, so there are no independent data on what gray-market vials contain. FDA’s recall records name no hexarelin product43. Health Canada notes that unauthorized injectable peptides have not been assessed for safety, efficacy or quality and may contain high-risk ingredients, additives or contaminants that may not be on the label22.

How it is sold

Sold online as ‘research use only’ powder, alone and in blends. On 7 October 2026 one US vendor listed hexarelin in 5 mg vials and in a 10 mg blend with CJC-1295 (Mod GRF 1-29), under the statement ‘All products are for laboratory research use only’7. FDA’s 2024 CJC-1295 briefing also found research-use-only powders combining CJC-1295 with hexarelin8. A 2026 medical review lists hexarelin among the peptides most commonly met in clinical practice and in online self-administration protocols5. In 2025 Health Canada listed hexarelin among unauthorized injectable peptides seized from or sold by Canada Peptide22.

Side effects and risks

  • Unknown long-term safety: the longest adult study we found lasted 16 weeks, and a 2026 review found no long-term safety data510.
  • Stress hormones and prolactin: single doses raise ACTH, cortisol and prolactin1314. In people with Cushing’s disease, the ACTH rise after hexarelin was about 7 times that after the natural releasing hormone CRH44.
  • Sleep: in 7 young volunteers, four nighttime doses reduced deep sleep compared with placebo15.
  • Fading effect: with daily use, the growth hormone response fell by about half, and it recovered after treatment stopped929.
  • Blood vessels: in isolated rodent hearts, hexarelin narrowed the heart’s blood vessels through CD3625, and in bypass patients it raised mean arterial pressure3.
  • Unregulated products: no independent tests of gray-market hexarelin are published21, and Health Canada warns that unauthorized injectable peptides may cause infection or allergic reactions and may contain contaminants22.
  • Legal and anti-doping risk: banned in sport at all times, and a breakdown product stays in urine after the drug itself has cleared; in Australia, possession without a prescription is illegal181937.

Documented harms

  • 13 Oct 2014The only report naming hexarelin in FDA’s adverse event system (FAERS), received 13 October 2014 from a US consumer, listed it as a concomitant drug alongside the anabolic steroid trenbolone, with pramipexole as the suspect drug. The reported problems were night sweats and insomnia, and the report was not marked serious. FAERS reports are unverified and cannot show which product, if any, caused an event45.
  • 1 Aug 2025Health Canada named hexarelin among the unauthorized injectable peptide drugs seized from and sold by Canada Peptide, warning that such products may cause infection, allergic reactions and other poor outcomes and may contain contaminants. The notice reports no case of harm22.

Questions people ask

Is hexarelin good for the heart?

It is not proven: in small studies, one dose into a vein briefly made the heart pump harder in healthy people, growth hormone-deficient patients and men having bypass surgery23. In severely weakened hearts the results were mixed, with no rise in dilated cardiomyopathy but a small rise in 5 patients whose heart damage came from blocked arteries231. We found no trial of repeated doses or of effects on symptoms or survival.

Is hexarelin the same as examorelin?

Yes. The anti-doping List names it ‘examorelin (hexarelin)’, and PubChem files the same molecule under examorelin1927.

Does hexarelin stop working over time?

Partly. In 12 older adults injected twice daily for 16 weeks, the growth hormone response fell by about 45% and recovered 4 weeks after stopping910. In short children dosed into the nose it roughly halved within a week, but growth still sped up29.

Does hexarelin build muscle or burn fat?

That has not been shown. In 12 older adults injected for 16 weeks, total body fat and lean body mass did not change significantly, though the study had no comparison group10, and a 2026 review found no controlled trials designed to test body composition5.

Hexarelin vs GHRP-6: what is the difference?

Hexarelin is GHRP-6 with one tryptophan swapped for the chemically more stable 2-methyl-tryptophan. Injected into a vein in adult rats, the two released similar amounts of growth hormone; under the skin, hexarelin gave a long-lasting release and was slightly more effective6.

Is hexarelin FDA approved?

No. Drugs@FDA lists no product containing hexarelin or examorelin426, and it appears on none of FDA’s compounding lists, so by our reading US pharmacies may not make it161734.

Is hexarelin banned in sports?

Yes. The 2026 and 2027 Prohibited Lists name examorelin (hexarelin) among growth hormone-releasing peptides banned at all times1920, and labs can find a breakdown product in urine after the drug itself has cleared37.

What is still unknown

  • Does the short-lived rise in heart pumping lead to any benefit with repeated doses? We found no human trial that tested this.
  • How long hexarelin itself stays in human blood; we found no published measurement.
  • Whether the heart effects seen in rats and mice, or the narrowing of heart blood vessels seen in isolated rodent hearts, occur in people is unknown.
  • We found no controlled trial that tested hexarelin for muscle, fat, recovery or aging outcomes.
  • We found no published independent purity or content testing of gray-market hexarelin.
  • UK, EU and Japanese status were not verified.
