Quick answers

What is it?
A lab-made chain of six amino acids, one of the first found to make the body release growth hormone14. It is sold online for muscle gain and fat loss, often in blends25.
Does it work?
It reliably raises growth hormone, and combined with a related hormone it works as a test for growth hormone deficiency6. Paired with another drug for stroke, it missed its main goal in a 188-patient trial7. We found no trial of muscle gain or fat loss2.
Is it safe?
Short studies reported few side effects attributed to it68, but it raises the stress hormone cortisol9. FDA flags possible immune reactions and effects on cortisol and blood sugar10. Long-term safety is unknown2.
Is it legal?
Not approved in the US3, and by our reading US pharmacies may not make it111213. Prescription-only in Australia, and illegal to hold without one14. Canada has seized it as an unauthorized drug15. Banned in sport at all times1617.
What is in the vials?
Finnrick, which publishes lab tests of peptide samples, had no GHRP-6 results on 7 October 202618. US pharmacies recalled 19 sermorelin blends containing it from 2012 to 2019 over sterility, 2 in the most serious class19. Seized powders have held look-alike copies20.

What it is

GHRP-6 (growth hormone-releasing peptide-6) is a lab-made peptide of six amino acids, two of them in mirror-image (D-) form4. Like ghrelin, the ‘hunger hormone’, it switches on the ghrelin receptor (also called the growth hormone secretagogue receptor), so the pituitary gland releases bursts of growth hormone (GH)2. In people, GH peaks 30 to 75 minutes after a dose by mouth or injection12. GHRP-6 also raises the stress hormones ACTH and cortisol9, and a 2026 review describes increased appetite as a characteristic effect2. In lab and animal studies it has protected heart and other tissues, partly through a second receptor, CD3621. It is not approved as a medicine in the US3.

GHRP-6 came from C.Y. Bowers’s group at Tulane University in the US, which found it in 1984 while designing peptides that release growth hormone22. The first report, which called it [His1,Lys6] GHRP, found that it released GH in rats, monkeys, lambs, calves and chicks without releasing other pituitary hormones4. By 1992 the group had shown that it also works by mouth in men and in short children, though with about 0.3% of the strength of an IV dose1. Stronger relatives, including GHRP-2 and hexarelin, were later built from it23. It is now understood to act on the receptor for ghrelin, a hunger hormone discovered in 1999222. Researchers have since developed it as a tissue-protecting drug: CIGB-500 for heart attacks24, and a combination with epidermal growth factor (EGF) for stroke, tested in a trial registered in Cuba8 and in a larger follow-up trial7.

Benefits: what the research shows

GHRP-6 reliably raises GH in people. It works into a vein, by mouth and into the nose, but a dose swallowed has only about 0.3% of the activity of an IV dose126. Combined with GHRH, the body’s own GH-releasing hormone, a single IV dose separated 125 adults with pituitary disease from 125 healthy adults (mean GH peaks of 4.1 vs 59.2 µg/L) without side effects6. It also acts beyond GH: in a sleep study, four IV doses of 50 µg raised GH, ACTH and cortisol and increased light (stage 2) sleep compared with placebo9, and a nasal dose raised GH while a dose by mouth had no effect26. In 7 healthy women aged 65 to 82, GHRP-6 by mouth twice daily for 4 days kept its GH effect but did not significantly raise IGF-1, the blood marker of GH action27. In 10 healthy men, GHRP-6 raised insulin and blood sugar only when the GH receptor was blocked by the drug pegvisomant and the men had not fasted28. The largest trials gave GHRP-6 together with epidermal growth factor (EGF), into a vein twice daily for 7 days, after an acute ischemic stroke. In a randomized trial of 36 patients, serious adverse events occurred in 9 of 16 controls and 5 of 20 treated patients, and two events in one treated patient were attributed to the treatment8. The follow-up phase III trial, COURAGE-2, randomized 188 patients and missed its primary endpoint: disability, daily functioning and survival did not differ overall, although a subgroup of 27 patients with severe stroke did better on treatment7. Neither trial was blinded, and because GHRP-6 was always given with EGF, they cannot show what GHRP-6 does on its own. In pigs, GHRP-6 cut the size of heart-attack damage by 78%29, but a 2026 review notes that these heart-protection findings come from animal and lab models without confirmatory human trials2. We found no trial of GHRP-6 for muscle gain, fat loss, recovery or anti-aging; a 2026 review calls the evidence for body-composition changes limited2. The closest is a chart review of 105 men on testosterone, seeking more lean mass and less fat, who were prescribed GHRP-6 with GHRP-2 and sermorelin; in the 14 who met strict criteria IGF-1 rose, and the abstract reports no body-composition results30. We rate GHRP-6 B because controlled human trials with public results exist, but none tested the uses it is sold for.

Doses studied

Doses below are what studies used or what a label says. They are not advice, and gray-market products may not contain what their label says.

In human studies

Studies in adults gave one IV dose of 1 µg/kg combined with GHRH 1 µg/kg for diagnosis6; four IV doses of 50 µg in a sleep study9; 1 µg/kg with or without pegvisomant28; and one IV dose of 100 to 400 µg/kg to measure drug levels31. By mouth, studies used single doses of 100 or 300 µg/kg in men and 300 µg/kg in children137, and a 300 µg/kg test dose in older women before and after 4 days of twice-daily treatment, at a dose the abstract does not state27. A sleep study compared 300 µg/kg by mouth with 30 µg/kg into the nose or under the tongue26. The stroke trials gave 3.5 or 5 mg IV with EGF twice daily for 7 days78. These studies measured hormones, sleep, drug levels or stroke recovery, not muscle or fat2.

In animal studies

Pigs: one 400 µg/kg dose in a heart-attack model cut infarct mass by 78% and thickness by 50% vs saline29. Dogs: daily IV doses of CIGB-500 (GHRP-6) at 300 to 2000 µg/kg for 28 days caused only temporary, reversible signs such as drooling, inactivity and a slower heart rate, and the highest dose was the no-observed-adverse-effect level24. Rats: injections under the skin once or twice daily for 9 or 25 days increased weight gain in young rats4.

What users report Anecdotal · not validated · not a recommendation

A 2026 medical review compiled bodybuilding-forum reports: 100 to 300 µg under the skin or into muscle, 2 or 3 times a day2.

Half-life
About 2.5 hours after a dose into a vein in healthy men (an older method gave 20 minutes)31In 9 healthy men given one IV dose of 100, 200 or 400 µg/kg and measured by mass spectrometry, the distribution half-life was 7.6 minutes and the elimination half-life 2.5 hours, and levels fell to the lowest measurable amount (5 ng/mL) by 12 hours31. A 1992 study that measured GHRP-6 with an antibody-based assay reported a serum half-life of 20 minutes after oral and IV doses; after a dose by mouth, GH returned to baseline within 150 to 180 minutes1.
Routes
  • Intravenous injection (hormone tests, sleep studies and the stroke trials)
  • Injection under the skin or into muscle (the usual gray-market routes)
  • By mouth (tested in adults and children)
  • Into the nose or under the tongue (tested in healthy men)

Quality and the gray market

What testing finds

Finnrick, a service that publishes lab results for peptide samples, listed no GHRP-6 tests and no rated vendors on 7 October 202618, so there are no independent data on what gray-market vials contain. FDA’s recall records name GHRP-6 in 19 compounded sermorelin products recalled by 8 US pharmacies between 21 May 2012 and 23 May 2019, all for sterility problems. Two were Class I, the most serious class: NuVision Pharmacy’s sermorelin/GHRP-6 vials in 2013, after an FDA inspection, and Town and Country Compounding’s in 2016, after contamination with the bacterium Bacillus circulans was found19. Doping-control chemists found glycine-extended GHRP-6, a look-alike with an extra amino acid, in powders seized by Danish customs20.

How it is sold

Sold online as ‘research use only’ powder, alone and in blends. On 7 October 2026 one US vendor listed GHRP-6 in 5 mg and 10 mg vials and in 4 blends with CJC-1295 or Mod GRF 1-29, sermorelin or GHRP-2, under the statement ‘All products are for laboratory research use only’5. FDA’s 2024 CJC-1295 briefing also found research-use-only powders combining CJC-1295 with GHRP-634. Some doctors have prescribed it with GHRP-2 and sermorelin30, and FDA recall records show US compounding pharmacies making sermorelin blends with GHRP-6 from at least 2012 to 201919. In 2025 Health Canada warned about unauthorized injectable GHRP-6 seized from or sold by Canada Peptide15, and in 2026 Australia’s regulator went to court against a company it says unlawfully advertised GHRP-635.

Side effects and risks

  • Unknown long-term safety: almost all human studies gave one dose or a few days of treatment, and a 2026 review found no long-term safety data2.
  • Stress hormones: GHRP-6 raised ACTH and cortisol in healthy men9, and FDA lists a possible effect on cortisol among its safety concerns10.
  • Blood sugar: FDA cites possible rises in blood glucose from lower insulin sensitivity10; in one small study, insulin and glucose rose only when the GH receptor was also blocked by a drug28.
  • Immune reactions and impurities: FDA warns that GHRP-6 may provoke immune responses by some routes because of clumping and peptide impurities10.
  • Serious events in stroke trials: in COURAGE-2, severe adverse events occurred in 30 of 95 treated patients and 18 of 93 controls, a difference that was not statistically significant, and none was judged treatment-related7.
  • Contaminated products: compounded sermorelin/GHRP-6 products were recalled for sterility problems, including two Class I recalls, one after bacterial contamination was found19.
  • Look-alike products: powders seized by Danish customs contained glycine-extended versions of GHRP-6 and related peptides20.
  • Legal and anti-doping risk: banned in sport at all times and found in urine 23 hours after one nasal dose; in Australia, possession without a prescription is illegal141636.

Documented harms

  • 15 Apr 2013NuVision Pharmacy in Dallas, Texas, recalled 122 vials of compounded sermorelin/GHRP-6 (3 mg/3 mg) in a Class I recall, the most serious class, for lack of sterility assurance and quality-control concerns found during an FDA inspection19.
  • 1 Aug 2016Town and Country Compounding and Consultation Services in Ridgewood, New Jersey, recalled 12 vials of sermorelin/GHRP-6 injection in a Class I recall after microbial contamination, identified as Bacillus circulans, was found19.
  • 10 Oct 2016Five US consumer reports to FDA’s adverse event system (FAERS), received from 10 October 2016 to 1 December 2016, involved compounded sermorelin products that also contained GHRP-6 and GHRP-2. They included a 64-year-old woman hospitalized with acute pancreatitis and a 39-year-old man hospitalized with pain and infection who needed repeated surgery; both reports were marked life-threatening. The other three described hives, itching, rash, burning, pain or fever. FAERS reports are unverified and cannot show which product, if any, caused an event38.
  • 2024In a phase I/II stroke trial of EGF plus GHRP-6, two serious adverse events in one treated patient were attributed to the treatment; the abstract does not say what they were. Serious adverse events overall were less frequent with treatment (5 of 20) than with standard care (9 of 16)8.
  • Oct 2026In the COURAGE-2 stroke trial, severe adverse events occurred in 48 of 188 patients: 30 of 95 given EGF plus GHRP-6 and, by subtraction, 18 of 93 on standard care (odds ratio 1.92, 95% CI 0.98 to 3.77). The investigators judged none treatment-related7.

Questions people ask

Is GHRP-6 FDA approved?

No. Drugs@FDA lists no product containing it, and by our reading FDA’s compounding rules do not let US pharmacies make it3111213.

Does GHRP-6 raise cortisol and blood sugar?

It raised ACTH and cortisol in healthy men9. FDA lists possible effects on cortisol and blood sugar among its safety concerns10, but in one small study insulin and glucose rose only when the growth hormone receptor was also blocked by a drug28.

Has GHRP-6 been tested in stroke patients?

Yes, but only together with another drug, epidermal growth factor. A 188-patient trial missed its main goal, with no overall difference in disability or survival, though a subgroup of 27 patients with severe stroke did better7.

Is GHRP-6 banned in sports?

Yes. The 2026 and 2027 Prohibited Lists name GHRP-6 among growth hormone-releasing peptides banned at all times1617, and a lab detected it in urine 23 hours after one nasal dose36.

GHRP-6 vs GHRP-2: what is the difference?

Both are lab-made chains of six amino acids that make the body release growth hormone, and GHRP-2 was developed from GHRP-6 as a stronger version2223. GHRP-2 is approved in Japan as a one-dose test for growth hormone deficiency39; GHRP-6 is not authorized in the US or Canada315.

What is CIGB-500?

CIGB-500 is a development product whose active ingredient is GHRP-6, meant to limit heart damage during a heart attack24. GHRP-6 reduced heart-attack damage in pigs29, but those findings have not been confirmed in human trials2.

What is still unknown

  • Does GHRP-6 add anything to EGF in stroke? Both trials gave the two drugs together, without blinding.
  • Published half-life estimates differ: 20 minutes with an antibody-based assay in 1992 vs 2.5 hours with mass spectrometry in 2013.
  • We found no controlled trial that tested GHRP-6 alone for muscle, fat, recovery or aging outcomes.
  • Whether repeated use raises blood sugar in people not taking a GH-receptor blocker is unknown.
  • No independent purity or content testing of gray-market GHRP-6 has been published.
  • UK and EU status, and whether GHRP-6 or CIGB-500 is approved in Cuba, were not verified.
